<strong>The</strong> set <strong>of</strong> methods and approaches to cl<strong>in</strong>ical and economic evaluation that have come to comprise<br />

<strong>HTA</strong> <strong>in</strong> developed economies developed alongside (and were shaped by) the particular context and<br />

issues encountered <strong>in</strong> those health systems. <strong>The</strong> UK NICE, for example, could be thought <strong>of</strong> as a<br />

particular response to issues relevant to the English NHS. <strong>The</strong> NHS is characterised by public<br />

fund<strong>in</strong>g, with most health care be<strong>in</strong>g ‘free at the po<strong>in</strong>t <strong>of</strong> demand’, but with an <strong>in</strong>creas<strong>in</strong>g shift<br />

toward private sector provision. Develop<strong>in</strong>g economies and emerg<strong>in</strong>g markets typically comprise<br />

more complex and varied f<strong>in</strong>anc<strong>in</strong>g and provision arrangements, and are likely to have different<br />

sorts <strong>of</strong> issues. <strong>The</strong>re is more likely to be a mix <strong>of</strong> public and private <strong>in</strong>volvement <strong>in</strong> both f<strong>in</strong>anc<strong>in</strong>g as<br />

well as provision <strong>of</strong> health care. <strong>The</strong> way <strong>in</strong> which the private and public sectors arrangements<br />

<strong>in</strong>teract may create specific challenges for the role <strong>of</strong> <strong>HTA</strong>. For example, <strong>in</strong> Brazil it is not clear how<br />

the use <strong>of</strong> <strong>HTA</strong> by CITEC impacts on the private sector.<br />

However, the <strong>HTA</strong> methods and processes they are likely to consider adopt<strong>in</strong>g come ‘fully formed’ –<br />

NICE’s processes, for example, have been particularly effectively dissem<strong>in</strong>ated. In Taiwan there is<br />

use <strong>of</strong> drug appraisals by NICE and other <strong>HTA</strong> bodies around the world, although it is not clear<br />

exactly how they are used and comb<strong>in</strong>ed to reach a conclusion. While NICE’s approach to technical<br />

appraisal may well be appropriate to some emerg<strong>in</strong>g markets, simply transferr<strong>in</strong>g those outcomes or<br />

<strong>in</strong>deed those processes <strong>in</strong> toto raises challenges <strong>in</strong> terms <strong>of</strong> local data, expertise, and resourc<strong>in</strong>g<br />

(Chalkidou et al., 2010). Moreover, there is a risk that simply adopt<strong>in</strong>g <strong>in</strong>fluential <strong>HTA</strong> processes<br />

developed elsewhere leads <strong>HTA</strong> down a path which does not meet the particular needs <strong>of</strong> local<br />

health care systems, and does not reflect locally-­‐relevant values and priorities. While there have<br />

been attempts to develop key pr<strong>in</strong>ciples for the conduct <strong>of</strong> <strong>HTA</strong> (Drummond et al., 2008), there is a<br />

lack <strong>of</strong> consensus among <strong>HTA</strong> organisations <strong>in</strong>ternationally about the importance or relevance <strong>of</strong><br />

some <strong>of</strong> these pr<strong>in</strong>ciples (Neumann et al., 2010), as we discuss below. What is clear is that before<br />

adopt<strong>in</strong>g <strong>HTA</strong>, countries should be clear about the problems which <strong>HTA</strong> is <strong>in</strong>tended to address, and<br />

should develop or adapt <strong>HTA</strong> processes to achieve that.<br />
