Annex: Note on the Experiment <strong>in</strong> N<strong>in</strong>gxia Prov<strong>in</strong>ce<br />

In N<strong>in</strong>gxia prov<strong>in</strong>ce there is a collaboration between the prov<strong>in</strong>cial government and the central<br />

government for a five year experiment 2009-­‐2013 to structure the health care system <strong>in</strong> order to<br />

make the best use <strong>of</strong> the funds. <strong>The</strong> objective is to identify most prevalent health problems<br />

<strong>in</strong>clud<strong>in</strong>g:<br />

<br />

<br />

chronic (e.g. hypertension, and diabetes),<br />

<strong>in</strong>fectious (e.g. TB, Hepatitis B, and dysentery) and<br />

common ailments (e.g. the common cold, UTI, and gastritis)<br />

Improvements <strong>in</strong> delivery to tackle these health problems <strong>in</strong>clude:<br />

Develop<strong>in</strong>g cl<strong>in</strong>ical protocol;<br />

Def<strong>in</strong><strong>in</strong>g the role and function <strong>of</strong> the three-­‐tiered delivery system: village cl<strong>in</strong>ics, THCs,<br />

county hospitals;<br />

Design<strong>in</strong>g provider payment method to <strong>in</strong>centivise providers to follow the best-­‐practice,<br />

<strong>in</strong>clud<strong>in</strong>g referral. This raises the question as to how to re-­‐tra<strong>in</strong> and/or reta<strong>in</strong> qualified<br />

village doctors?<br />

Design<strong>in</strong>g benefit package to reduce f<strong>in</strong>ancial barrier to access care and to motivate patients<br />

to use primary-­‐based care<br />

Us<strong>in</strong>g technology (e.g. mobile phone) to improve patient compliance.<br />

Conduct<strong>in</strong>g patient education program on prevention, healthy lifestyle and best-­‐practice<br />

care<br />

Creat<strong>in</strong>g a purchaser who acts <strong>in</strong> the best <strong>in</strong>terest <strong>of</strong> the <strong>in</strong>sured population.<br />

<strong>The</strong>re are 2 <strong>in</strong>tervention counties, 3 comparison counties, a total <strong>of</strong> 1.8 million people. <strong>The</strong> study<br />

design is with<strong>in</strong> county: blocked randomization across townships and villages with phased <strong>in</strong><br />

<strong>in</strong>terventions. It covers five years: 2009-­‐2013, and uses:<br />
