<strong>Services</strong> Provided<br />

TEAM<strong>Health</strong>’s services are provided within a model of recovery focussed<br />

psychosocial rehabilitation <strong>and</strong> within the framework of the National <strong>Mental</strong><br />

<strong>Health</strong> Plan <strong>and</strong> St<strong>and</strong>ards. TEAM<strong>Health</strong> works closely with other specialist<br />

service providers such as Top End <strong>Mental</strong> <strong>Health</strong> <strong>Services</strong> (TEMHS), drug/<br />

alcohol services <strong>and</strong> other non-government organisations.<br />

TEAM<strong>Health</strong> provides a wide range of services for people with a mental illness.<br />

These include:<br />

• The Community Housing Program which provides medium to long term<br />

housing options for people <strong>and</strong> families affected by mental illness who have<br />

the capacity to live independently. Referrals are accepted from any source.<br />

• The Day-to-Day Living (D2DL) Program which provides drop-in <strong>and</strong> structured<br />

socially based activity programs with a focus on return to work. Referrals are<br />

accepted from any source.<br />
