22. Networks (formal &<br />
informal)<br />
Office of Housing: SHASP have a formal network relationship with<br />
the OOH. We have a specific OOH referral form for SHASP and<br />
rely on OOH to identify tenancies at risk or new tenancies moving<br />
into OOH and make referrals to SHASP accordingly.<br />
SAM – Support Allocation Meeting: SHASP along with<br />
representatives from other <strong>HomeGround</strong> Collingwood Programs<br />
meet weekly to discuss referrals received via the Intake worker for<br />
support and allocate them accordingly to the most appropriate<br />
program.<br />
Neighbourhood Justice Centre: SHASP have a good informal<br />
working relationship with the NJC, in particular the Legal Aid<br />
lawyers and Financial Counsellors.<br />
Community Health Centres: SHASP frequently work to refer clients<br />
to various parts of the COY Community Health Centres and take<br />
referrals from them as well.<br />
Clarendon Clinic: SHASP share several clients with Clarendon<br />
Clinic and work together to negotiate housing outcomes for mutual<br />
clients.<br />
23. Disabled access <strong>HomeGround</strong> has disability access at our office via an electronic<br />
lift.<br />
24. Transport <strong>HomeGround</strong> Collingwood is accessible by tram and bus located<br />
nearby. SHASP is an outreach support program which provides<br />
vehicle access for clients to get to appointments, VCAT or drop<br />
clients home etc.<br />
25. Culture and<br />
Languages<br />
SHASP utilise the “On-Call” Interpreting & Translation <strong>Services</strong> for<br />
all clients requiring an interpreter. This can be via phone or in<br />
person at an appointment, VCAT or home visit.<br />
<strong>HomeGround</strong> have Client <strong>Information</strong> Kits that are available in<br />
various languages and further SHASP have introduction letters<br />
available in various translations as well.<br />
