12. Hours open St Vincent‟s at Home – office 8am to 4.30pm Mon- Friday<br />
- service 7 days a week 7.30am-9.30pm.<br />
13. Costs involved HACC visit fees are charged for community nursing and the patient<br />
is responsible for purchasing their medications and wound care<br />
dressings.<br />
No fees for Hospital in the Home or The Cottage and all<br />
consumables provided.<br />
14. Target Group St Vincent‟s at Home- Any client of St Vincent‟s Hospital requiring<br />
nursing care.<br />
The Cottage- Any client experiencing homelessness with acute<br />
health issues. Diversion from Emergency or an inpatient stay.<br />
15. Eligibility St Vincent‟s at Home is for clients of St Vincent‟s Hospital.<br />
Patients at the Cottage need to be medically stable, cannot drink<br />
alcohol or take illicit drugs, need to be ambulant and able to<br />
communicate effectively.<br />

St Vincent‟s at Home is a community nursing service.<br />
The Cottage provides short term health care for clients<br />
experiencing homelessness. Diversion from Emergency or an<br />
