2. Brief History of Service The Homeless Persons Program (HPP) was established in 1978 to<br />

provide a nursing service to people experiencing homelessness.<br />

HPP became a part of the Royal District Nursing Service (RDNS) in<br />

1988 and since then has experienced its most significant growth.<br />

RDNS HPP employs 34 community health nurses who provide<br />

direct care to individuals and families who are homeless or at risk.<br />

RDNS HPP co locates direct care staff with 22 agencies, spanning<br />

a broad range of service, who work with or are located near<br />

populations of people experiencing homelessness<br />

8. HACC Service Since 1988, RDNS HPP funding has increased from $136,000 to<br />

over $3 million – the majority of funding is received via the<br />

Federal/State Home and Community Care funding arrangement<br />

(HACC), with a quarter of funding coming from contracted services<br />

via the homeless sector.<br />

RDNS HPP assertively seeks to take healthcare to people<br />

experiencing homelessness. Rather than waiting for people to<br />

contact the program, staff outreach into environments where<br />

homeless live and gather (nursing clinics, crisis accommodation,<br />

rooming houses or even parks).<br />

RDNS HPP nurses provide primarily nursing care as well as<br />

advocacy, assessment and case management to people<br />

experiencing homelessness.<br />

9. Other <strong>Services</strong> in<br />

Organisation<br />

RDNS general also provides in home care to clients in Yarra and<br />

when appropriate the RDNS general nurses will share care with<br />

HPP if daily visits are required for clinical care.<br />

10. Funding Sources HACC funding and HARP funding one day per week at St.<br />

Vincent‟s Referrals position to deliver health care to people<br />

experiencing or who are at risk of homelessness in the community.<br />
