Predominantly home-based outreach, some service delivery at other<br />

services and transportation is provided. Some centre-based<br />

services provided, mostly the allied health services.<br />

I’ve made a referral?<br />

Normally the referral is completed over the phone via the intake<br />

worker, or the doctor will complete it and refer the client on to us.<br />

Currently this process is under review, however usually referrals are<br />

discussed at weekly meetings and allocated to appropriate workers<br />

and services. Can also be responsive to immediate demand.<br />
