3. Certificate of Insurance, naming <strong>Loudoun</strong> <strong>Water</strong> as additional insured. Certificate must<br />
cite the project in question, using its <strong>Loudoun</strong> <strong>Water</strong> Project ID Number.<br />
4. materials submittal including shop drawings, as applicable<br />
5. payment of an Inspection Fee; and for sewers, a Closed Circuit Television (CCTV)<br />

6. conveyance and recordation of supporting easement<br />
F. As-built Survey and Record Drawing Fee<br />
As stated in the preceding Section 2.12 titled Project Closeout, a fee for this service may be<br />
paid at time of construction permitting, and must be received prior to the facilities receiving<br />
the status Beneficial Use.<br />
G. Revisions to Approved Construction Plans<br />
Revisions to approved plans require:<br />
1. UER Form<br />
2. Review Fee<br />
3. letter listing the impacted sheet numbers and the nature of design modifications<br />
4. one copy of each plan sheet that has been revised, with the proposed changes highlighted.<br />
Each sheet must have a revision block, reflecting each date of revision since initial<br />
approval.<br />
Revisions to approved construction plans may require subsequent submissions, additional<br />
fees, revised bonds, and/or revised easements, depending on the nature of the revision. Once<br />
the revision is to be approved, provide three copies of each plan sheet that has been revised.<br />
3.4 Calculation of Review Fees<br />
A. Pipeline Projects<br />
The Review Fee is to be calculated based on the lengths of pipeline, in accordance with the<br />
Utility Extension Request Form (UER). The initial payment covers the first and second<br />
reviews. After the second review, if the plan requires further revisions, a Subsequent Review<br />
Fee is to be paid at time of each submission. The UER Form submitted with each package<br />
must reflect the lengths upon which the fee has been based, and these must match current<br />
construction plans.<br />
For revisions to previously approved plans, the fee may be based simply on lengths of pipe<br />
that have been the subject of the current revision.<br />
