SECTION IV<br />

COST PROPOSAL/FORMS<br />

1.0 Prices Required & Related Information<br />

Proposers shall submit a price per Trip (“Trip” defined below).<br />

TRIP – A single one-way <strong>ParaTransit</strong> ride from pick-up<br />

location to drop-off location for one Eligible Rider.<br />

The Provider will be paid for each “Trip” except that the <strong>County</strong> will pay half of<br />

the per Trip rate established under the contract associated with this RFP for “No-<br />

Show” or “Cancelled-at-Door” trips. See Section III, 7.0 (Definitions). For all<br />

No-Show and Cancelled-at-Door trips, the driver must note a “Point of Site”<br />

(POS) on the Driver Log, i.e., note a landmark as proof of arrival.<br />

The prices offered will be entered on the Price Offer Forms attached hereto.<br />

2.0 Selection Investigation Criteria<br />

The <strong>County</strong> may make such investigation as it deems necessary to determine<br />

the ability of the proposer to perform the work, and the proposer shall furnish all<br />

information and data for this purpose as may be requested. WC reserves the<br />

right to reject any proposal if the evidence submitted by, or the investigation of<br />

such proposer fails to satisfy WC that he is properly qualified to carry out the<br />

obligations of the contract and to complete the contemplated work.<br />

3.0 Criteria for Disqualification<br />

• Failure to properly complete all forms.<br />

• Failure to have a minimum of 3 years experience providing demand<br />

responsive service (i.e. service which is scheduled less than 24 hours in<br />

advance including a minimum of ten vehicles in service on a weekday) or a<br />

minimum of 3years providing public transit service.<br />

• Failure to submit a drug and alcohol testing plan or which submit one that<br />

does not comply with the Omnibus Transportation Employee Testing Act of<br />

1991 and specifically 49 CFR part 653, 49 CFR part 654 and 49 CFR part 40<br />

(as amended).<br />
