System of Care: An organizational philosophy and framework that involves collaboration across agencies, families, and youth for the purpose of improving access and expanding the array of coordinated community-based, culturally and linguistically competent services and supports for children and youth who are diagnosed with a serious emotional disturbance and their families (Stroul & Friedman, 1986). System of Care Principles: Accessible, collaborative, community based, culturally and linguistically competent, family driven, individualized, interagency, least restrictive, youth guided. Transition-Age Youth: Youth transitioning from adolescence to adulthood, ages 14 years and older. Wraparound: ―Ateam-based, collaborative process for developing and implementing individualized care plans for children with severe disorders and their families…The values associated with wraparound specified that care was to be strengths based, culturally competent, and organized around family members’ own perceptions of their needs and goals‖ (Walker & Bruns, 2006). Youth-Guided Care: Youth are engaged as equal partners in creating systems change in policies and procedures at the individual, community, State, and national levels.
