CHALLENGES IN DE-IDENTIFYING FILM-SCREEN MAMMOGRAMS PRIOR TO BREAST
DENSITY ASSESSMENT FOR THE NOVICE PRACTITIONER
Introduction: Measurement of breast density on mammograms presents many challenges. Film-screen
mammograms require conversion to digital format prior to assessment with computer software, but
contain burned-in areas of identifying information that should be removed to protect participant privacy.
We are undertaking a retrospective linkage study in at least 4,000 women between the lower Hunter New
England (NSW Australia) BreastScreen and the NSW 45 and Up Study to examine longitudinal changes
in breast density and breast cancer risk. The BIRADS density categories are not routinely employed in
Australia hence density will be assessed via the Cumulus software and/or visual methods. A simple,
efficient and inexpensive method to de-identify thousands of film-screen mammograms is needed.
Coverage of identifying areas on film with standard sticky labels during digitisation on an Array 2905HD
laser scanner is not effective due to the strength of the laser. Our aim was to evaluate a range of software
programs available for this purpose.
ABSTRACTS
Methods: Software capable of quickly and easily batch de-identifying digital images in DICOM format
was sought via general internet searches using the terms ‘DICOM anonymizer burned in’; searches in
PubMed using similar terms were also undertaken. Software was evaluated by: 1) its capacity to easily
batch de-identify DICOM images with burned-in identifying areas (i.e. digitised film-screen
mammograms), 2) the ability to easily remove DICOM header information from digital mammograms,
and 3) retention of detail and compatibility with the Cumulus software post-processing.
Results: More than 60 free DICOM viewers and 20 de-identifiers/anonymisers were identified
(dclunie.com). Searches in PubMed found two useful, recent (
