NSTA 2009 <strong>New</strong> <strong>Orleans</strong> <strong>National</strong> Conference<br />

Professional Development Documentation Form<br />

All attendees can evaluate concurrent teacher and exhibitor sessions, NSTA symposia, professional development institutes, and<br />

the <strong>Science</strong> Assessment, Linking <strong>Science</strong> and Literacy, and <strong>Science</strong> and English Language Learners conference while simultaneously<br />

tracking professional development certification (based on clock hours). Use this form to keep track of sessions/events you<br />

attended at the <strong>New</strong> <strong>Orleans</strong> conference for which you did not receive a session evaluation form, either because the presenter<br />

did not provide one or because the activity is not being evaluated (field trips, short courses, featured speakers, the General<br />

Session, meetings, and exhibit hall visits).<br />

Beginning April 27, 2009, <strong>New</strong> <strong>Orleans</strong> transcripts can be accessed at http://ecommerce2.nsta.org/transcript/ by logging<br />

on with your <strong>New</strong> <strong>Orleans</strong> Badge ID*. Keep this form and use it to add the listed activities to your <strong>New</strong> <strong>Orleans</strong> transcript.<br />

Completed transcripts can be printed from this website and presented to an administrator who requires documentation of<br />

participation in the conference. All information in these transcripts will be maintained (and can be accessed) indefinitely as<br />

part of an attendee’s individual profile.<br />

Be sure to place session evaluation forms in the designated drop-off boxes no later than 12:30 PM on Sunday, March 22.<br />
