That the GAL is appointed, pursuant to I.C. 31-15-6-1, et seq., and/or other appropriatesection(s) <strong>of</strong> the <strong>Indiana</strong> Code and/or <strong>Local</strong> <strong>Rule</strong>s, for the following minor children <strong>of</strong> theparties:namedate <strong>of</strong> birthnamedate <strong>of</strong> birthnamedate <strong>of</strong> birth2. That this appointment <strong>of</strong> the GAL shall become effective upon payment to the GAL <strong>of</strong>the required monetary retainer and the GAL’s acceptance <strong>of</strong> the same. The issue <strong>of</strong> the retainerpayment to the GAL shall be set for a review hearing within days. Attorney forshall coordinate such hearing date and assure that the same occurs if theGAL has not been paid in a timely manner.Said monetary retainer shall be in the amount <strong>of</strong> $. The GAL is herebyauthorized to bill the parties for services rendered at the GAL’s usual rate, with theunderstanding that said usual rate may be reduced to the county’s service rate if any remainder <strong>of</strong>the GAL’s bill is later submitted to Porter County for payment.25
