3400.40 Entry Form. Every filing subsequent to the original complaint, shall beaccompanied by an entry form, in duplicate, which shall contain the title and number <strong>of</strong> the case,the date, and the exact entry to appear on the Chronological Case Summary. The entry form shallbe typewritten or legibly printed, and shall be signed by counsel. Hearing dates on filingsrequiring Court action shall be obtained from the Judge's staff. Hearing dates shall be inserted onthe entry at the time <strong>of</strong> filing. All entries will be examined and approved by the Judge to whomthe case is assigned, or by any other sitting Judge, if the Judge to whom the case is assigned isunavailable, prior to inclusion in the Court's entries.3400.50 Service <strong>of</strong> Copies on Counsel and Unrepresented Parties. Every filingrequired to be served by T.R. 5 shall be served on all counsel <strong>of</strong> record either before it is filed or onthe day it is filed with the Court. A copy <strong>of</strong> the entry form <strong>of</strong> the filing shall also be served on allcounsel <strong>of</strong> record whenever the entry is the appearance <strong>of</strong> counsel or contains a setting for a Courthearing date. All proposed forms <strong>of</strong> order shall be submitted in sufficient number thatdistribution may be made to all parties.3400.60 Routine Entries. Entries, either routine in nature or uncontested including,for example, those setting or continuing a hearing, shall be set out on an entry form only, whichshall contain the concise substance <strong>of</strong> the entry.3400.70 Electronic Facsimile Filing. Only parties who have agreed to acceptservice by electronic facsimile transmission (FAX) are permitted to file by FAX. Any filing byFAX shall not exceed ten (10) pages. FAX filings should be made to the Court in which the caseis pending.LR64-AR00-3500CONTINUANCES AND SETTLEMENTS3500.10 Motions for Continuance are discouraged. Neither side is entitled to anautomatic continuance as a matter <strong>of</strong> right.3500.20 Motion for Continuance. Unless made during a hearing or trial, a Motionfor Continuance shall be made in writing, stating with particularity the grounds therefore and beverified, and shall state whether opposing counsel objects to the motion, and whether priorcontinuances have been requested by the moving party.(1) The Court may require any written Motion for Continuance to be signed by the partyrequesting the continuance in addition to the signature <strong>of</strong> the attorney so moving.(2) The Court may require the stipulation to continue the hearing <strong>of</strong> any pending matter tostate with particularity the grounds for the continuance and be signed by all attorneys <strong>of</strong>record.34
