The owner or operator subject to theinspection <strong>and</strong> maintenance requirements of subsection93102.10(a) <strong>and</strong> (b) shall prepare an operation <strong>and</strong> maintenanceplan. For major sources, the plan shall be incorporated byreference into the source's title V permit. The plan shall incorporatethe inspection <strong>and</strong> maintenance requirements <strong>for</strong> that device, ormonitoring equipment, as identified in Table 93102.10, orsubsection 93102.10(b) of this <strong>ATCM</strong> <strong>and</strong> include the followingelements:(1) A st<strong>and</strong>ardized checklist to document the operation <strong>and</strong>maintenance of the facility, the add-on air pollution controldevice, <strong>and</strong> the process <strong>and</strong> control system monitoringequipment; <strong>and</strong>(2) Procedures to be followed to ensure that equipment isproperly maintained. [To satisfy the inspection <strong>and</strong>maintenance requirements of this subsection, the owner oroperator may use applicable st<strong>and</strong>ard operating procedure(SOP) manuals, Occupational Safety <strong>and</strong> HealthAdministration (OSHA) plans, or other existing plans, providedthe alternative plans meet the requirements of thissubsection.](b)(c)Retain the O & M plan. The owner or operator shall keep thewritten operation <strong>and</strong> maintenance plan on record after it isdeveloped to be made available <strong>for</strong> inspection, upon request,during normal working hours.Changes to the O & M plan. Any changes made by the owner oroperator should be documented in an addendum to the plan. Inaddition, the owner or operator shall keep previous (i.e.,superseded) versions of the operation <strong>and</strong> maintenance plan onrecord to be made available <strong>for</strong> inspection, upon request, duringnormal working hours, <strong>for</strong> a period of 5 years after each revision tothe plan.32
Effective Date: October 24, 2007(d)Revisions to the O & M plan to address breakdowns. The owner or operator shall maintaininspection records to document that the inspection <strong>and</strong>maintenance requirements of section 93102.10 <strong>and</strong> Table93102.10, <strong>and</strong> the provisions of the operation <strong>and</strong> maintenanceplan required by section 93102.11 have been met. The record cantake the <strong>for</strong>m of a checklist <strong>and</strong> shall identify:(1) the device inspected,(2) the date <strong>and</strong> time of inspection,(3) a brief description of the working condition of the device duringthe inspection,(4) maintenance activities per<strong>for</strong>med on the components of the airpollution control system (i.e. duct work replacement, filter padreplacement, fan replacement, etc), <strong>and</strong>(5) actions taken to correct deficiencies found during theinspection.(b)(c)Per<strong>for</strong>mance test records. The owner or operator shall maintaintest reports documenting the conditions <strong>and</strong> results of allper<strong>for</strong>mance tests.Monitoring data records. The owner or operator shall maintainrecords of monitoring data required by section 93102.9 that areused to demonstrate compliance, including the date <strong>and</strong> time thedata are collected.(1) Cumulative rectifier usage records. Record the actualcumulative rectifier usage expended during each month of thereporting period, <strong>and</strong> the total usage expended to date.(2) Pressure drop. The owner or operator shall record thepressure drop once a week.33
