ConsentThe City of Burnside has a legal responsibility to comply with the Freedomof Information Act 1991. If consent has not been previously given in theService User Consent Form a Request for Information Form will need to becompleted and provided to the Manager Aged and CommunityDevelopment. Information will be provided in 3 business days to the personmaking the request. Forms are available on the Council website or bycontacting the City of Burnside Home and Community Care staff.Service users also have the right to withdraw consent to the release of theservice user’s and/or carer’s personal information.In an emergency, release of the service user’s information may occur to theservice user’s nominated carer, advocate, legal guardian or medicalprofessional(s) as recorded in the Service User Consent Form.Service users should contact the relevant Home and Community CareProgram Coordinator if they have any comments or complaints about theway staff are handling their information.
