<strong>Disaster</strong> <strong>and</strong> <strong>Emergency</strong> <strong>Preparedness</strong>: <strong>Guidance</strong> <strong>for</strong> SchoolsAt request gate: Greet <strong>and</strong> direct parents/guardians through the request process; Provide parents with student-family reunification <strong>for</strong>m (permit <strong>for</strong> release ofchild) to be filled out; Verify authorization on “student emergency contact” cards; Request identification. If parents or guardians are known to staff or positivelyidentified by student, this may be used in lieu of official identification, subject toapproval of administrator; Keep the top portion of the <strong>for</strong>m at the request gate filed in alphabetical order; Locate child using student schedule location roster <strong>and</strong> identify location in emergencyassembly area; Send a runner with the middle portion of the <strong>for</strong>m to locate the student in the assemblyarea; Give the bottom portion of the <strong>for</strong>m to the parent/guardian <strong>and</strong> direct them to thereunification gate; If a second person comes to find the same student, check request <strong>for</strong>m <strong>and</strong> directparent to the reunification gate <strong>for</strong> detailed in<strong>for</strong>mation.At reunion gate:Match request <strong>for</strong>m with student. Request identification. In the case of discrepanciesrequest adult to return to request gate.If a second person comes to find the same student, verify that the student waspicked up, when <strong>and</strong> by whom.The logistics branch leader’s responsibilities are:Immediately lock all external gates <strong>and</strong> doors—secure campus;Report to incident comm<strong>and</strong>er;As needed, mobilize individuals to obtain equipment <strong>and</strong> people to support theoperations branch;Monitor gates <strong>and</strong> open <strong>for</strong> emergency vehicles, <strong>and</strong> direct first responders to areaof need;Post signs as needed;Direct parents to the “request gate”;Check utilities <strong>and</strong> take action to minimize damage to school site;Assess damage to site <strong>and</strong> report findings to incident comm<strong>and</strong> center;Establish morgue area, if needed;36
