Residential: Intensive Residential Servicesd. All areas <strong>of</strong> the residential facility must be clean, safe, appropriately equipped, <strong>and</strong> furnished for the services delivered.e. The facility must comply with the Americans with Disabilities Act.f. The facility must maintain a written evacuation plan to be used in the case <strong>of</strong> fire or other disaster. An appropriate written certification <strong>of</strong> compliance must be obtainedindicating that all applicable fire <strong>and</strong> safety code requirements have been satisfied. Periodic fire <strong>and</strong> other safety drills must be conducted.g. Evacuation routes must be clearly marked by exit signs.h. The program must be responsible for providing physical facilities that are structurally sound <strong>and</strong> that meet all applicable federal, state, <strong>and</strong> local regulations forStaffingRequirementsClinicalOperationsadequacy <strong>of</strong> construction, safety, sanitation, <strong>and</strong> health.1. Residential Managers may be persons with at least 2 years experience providing MH or AD services <strong>and</strong> at least a high school diploma; however this person must bedirectly supervised by a licensed staff member (including LMSW, LAMFT, LAPC, or 4-year RN).2. Persons with high school diplomas, GEDs or higher, who have completed the parapr<strong>of</strong>essional training required for DBHDD contracted organizations <strong>and</strong> under thesupervision <strong>of</strong> a Residential Manager may perform residential services.3. A minimum <strong>of</strong> at least one (1) awake on-site staff 24/7.1. The organization must have a written description <strong>of</strong> the Intensive Residential Service <strong>of</strong>fered that includes, at a minimum, the purpose <strong>of</strong> the service; the intendedpopulation to be served; service philosophy/model, level <strong>of</strong> supervision <strong>and</strong> oversight provided; <strong>and</strong> outcome expectations for its residents.2. Intensive Residential Service assists those individuals with an intensive need for personal supports <strong>and</strong> skills training to restore, develop, or maintain skills in functionalareas in order to live meaningful lives in the community; develop or maintain social relationships, <strong>and</strong> participate in social, interpersonal, vocational, recreational orcommunity activities. Services must be delivered to consumers relevant to their individualized Recovery Plan.3. Intensive Residential Service must provide a minimum <strong>of</strong> 5 hours <strong>of</strong> skills training <strong>and</strong>/or support activities per week that relate to the individual’s IRP.Skills Training may include interpersonal skills training; coping skills/problem solving; symptom identification <strong>and</strong> management; cooking; maintaining a residence;using public transportation; shopping; budgeting <strong>and</strong> other needed skills training as identified in the IRP.Reporting <strong>and</strong>BillingRequirementsDocumentationRequirementsSupport Activities may include daily contacts by Intensive Residential Service staff daily to monitor physical <strong>and</strong> mental health needs; crisis intervention whenneeded; assistance with scheduling <strong>of</strong> medical <strong>and</strong> mental health appointments; the supervision <strong>of</strong> the self-administration <strong>of</strong> medications; transportation tomedical/dental/mental health/employment/recreational activities; participation in community activities; <strong>and</strong> other needed supports as identified in the IRP.Each month, the provider must submit a Monthly Residential Service Report developed by the <strong>Department</strong> that identifies the actual utilization <strong>of</strong> intensive residentialservices including amount spent, number <strong>of</strong> units occupied, <strong>and</strong> number <strong>of</strong> individuals served.1. The organization must develop <strong>and</strong> maintain sufficient written documentation to support the Intensive Residential Service for which billing is made. Thisdocumentation, at a minimum, must confirm that the individual for whom billing is requested was a resident <strong>of</strong> the Intensive Residential Service on the date <strong>of</strong> service.The individual’s record must also include each week’s programming/service schedule in order to document the provision <strong>of</strong> the required amount <strong>of</strong> skills training <strong>and</strong>support activities.2. Weekly progress notes must be entered in the individual’s record to enable the monitoring <strong>of</strong> the individual’s progress toward IRP <strong>and</strong> recovery goals.3. The record should include health issues <strong>and</strong> how they are being addressed; appointments for psychiatric <strong>and</strong> medical care that are scheduled for the consumer;attendance at other treatments such as addictive diseases counseling that staff may be assisting consumer to attend; assistance provided to the consumer to help himor her reach recovery goals; <strong>and</strong> the consumer’s participation in other recovery activities.4. Each note must be signed <strong>and</strong> dated <strong>and</strong> must include the pr<strong>of</strong>essional designation <strong>of</strong> the individual making the entry.FY2013 Provider Manual for Community <strong>Behavioral</strong> <strong>Health</strong> Providers Page 125
