The program utilizes methods, materials, settings, <strong>and</strong> outside resources appropriate to the developmental <strong>and</strong> cognitive levels, capabilities, age, gender, <strong>and</strong> culture<strong>of</strong> participants.5. The program utilizes methods, materials, approaches, activities, settings, <strong>and</strong> outside resources appropriate for <strong>and</strong> targeted to individuals with co-occurring disorders<strong>of</strong> mental illness <strong>and</strong> substance abuse <strong>and</strong> targeted to individuals with co-occurring developmental disabilities <strong>and</strong> substance abuse when such individuals arereferred to the program.6. The program conducts r<strong>and</strong>om drug screening <strong>and</strong> uses the results <strong>of</strong> these tests for marking participant’s progress toward goals <strong>and</strong> for service planning.7. The program is provided over a period <strong>of</strong> several weeks or months <strong>and</strong> <strong>of</strong>ten follows detoxification or residential services.8. This service must operate at an established site approved to bill Medicaid for services. However, limited individual or group activities may take place <strong>of</strong>f-site in naturalcommunity settings as is appropriate to each individual’s treatment plan. (Narcotics Anonymous (NA) <strong>and</strong>/or Alcoholics Anonymous (AA) meetings <strong>of</strong>fsite may beconsidered part <strong>of</strong> these limited individual or group activities for billing purposes only when time limited <strong>and</strong> only when the purpose <strong>of</strong> the activity is introduction <strong>of</strong> theparticipating individual to available NA <strong>and</strong>/or AA services, groups or sponsors. NA <strong>and</strong> AA meetings occurring during the SA Intensive Outpatient package may notbe counted as billable hours for any individual outpatient services, nor may billing related to these meetings be counted beyond the basic introduction <strong>of</strong> an individualto the NA/AA experience.).9. This service may operate in the same building as other services; however, there must be a distinct separation between services in staffing, program description, <strong>and</strong>physical space during the hours the SA Intensive Outpatient Services is in operation.10. Adequate space, equipment, furnishings, supplies, <strong>and</strong> other resources must be provided in order to effectively provide services <strong>and</strong> so that the program environmentis clean <strong>and</strong> in good repair. Space, equipment, furnishings, supplies, transportation, <strong>and</strong> other resources for participating individuals’ use within the Substance AbuseIntensive Outpatient package must not be substantially different from that provided for other uses for similar numbers <strong>of</strong> individuals.1. The program must be under the clinical supervision <strong>of</strong> a Level 4 or above who is onsite a minimum <strong>of</strong> 50% <strong>of</strong> the hours the service is in operation.2. Services must be provided by staff who are:a. Level 4 (LAPC, LMSW, CACII, CADC, CCADC <strong>and</strong> Addiction Counselor Trainee with supervision)b. Level 5 (Parapr<strong>of</strong>essionals, high school graduates) under the supervision <strong>of</strong> an Level 4 or above3. It is necessary for all staff who provide this “co-occurring capable” service to have basic knowledge in the Georgia DBHDD content areas in the Suggested BestPractices Principles <strong>and</strong> Staff Capabilities for Services Serving Individuals with Co-Occurring Disorders document included in this Provider Manual4. Programs must have documentation that there is one Level 4 staff (excluding Addiction Counselor Trainee) that is “co-occurring capable.” This person’s knowledgemust go beyond basic underst<strong>and</strong>ing <strong>and</strong> must demonstrate actual staff capabilities in using that knowledge for individuals with co-occurring disorders. Personneldocumentation should demonstrate that this staff person has received a minimum <strong>of</strong> 4 hours <strong>of</strong> training in co-occurring treatment within the past 2 years.5. There must be at least a Level 4 practitioner on-site at all times the service is in operation, regardless <strong>of</strong> the number <strong>of</strong> individuals participating.6. The maximum face-to-face ratio cannot be more than 12 individuals to 1 direct program staff based on average daily attendance <strong>of</strong> individuals in the program.7. The maximum face-to-face ratio cannot be more than 20 individuals to 1 SAP based on average daily attendance <strong>of</strong> individuals in the program.8. A physician <strong>and</strong>/or a Registered Nurse or a Licensed Practical Nurse with appropriate supervision must be available to the program either by a physician <strong>and</strong>/ornurse employed by the agency, through a contract with a licensed practitioner, or by written referral or affiliation agreement with another agency or agencies that <strong>of</strong>fersuch services.a. The physician is responsible for addiction <strong>and</strong> psychiatric consultation, assessment, <strong>and</strong> care (including but not limited to ordering medications <strong>and</strong>/orlaboratory testing) as needed.b. The nurse is responsible for nursing assessments, health screening, medication administration, health education, <strong>and</strong> other nursing duties as needed.9. Level 4 staff may be shared with other programs as long as they are available as required for supervision <strong>and</strong> clinical operations <strong>and</strong> as long as their time isappropriately allocated to staffing ratios for each program.FY2013 Provider Manual for Community <strong>Behavioral</strong> <strong>Health</strong> Providers Page 134
1. It is expected that the transition planning for less intensive service will begin at the onset <strong>of</strong> these services. Documentation must demonstrate this planning.2. A consumer may have variable length <strong>of</strong> stay. The level <strong>of</strong> care should be determined as a result <strong>of</strong> consumers’ multiple assessments. It is recommended thatindividuals attend at a frequency appropriate to their level <strong>of</strong> need. Ongoing clinical assessment should be conducted to determine step down in level <strong>of</strong> care.3. Each consumer should participate in setting individualized goals for themselves <strong>and</strong> in assessing their own skills <strong>and</strong> resources related to sobriety, use/abuse, <strong>and</strong>maintaining recovery. Goals are set by exploring strengths <strong>and</strong> needs in the consumer’s living, learning, social, <strong>and</strong> working environments. Implementation <strong>of</strong>services may take place individually or in groups.4. Each consumer must be provided assistance in the development/acquisition <strong>of</strong> needed skills <strong>and</strong> resources necessary to achieve sobriety <strong>and</strong>/or reduction in abuse<strong>and</strong> maintenance <strong>of</strong> recovery.5. Substance Abuse Intensive Outpatient Package must <strong>of</strong>fer a range <strong>of</strong> skill-building <strong>and</strong> recovery activities within the program.6. The following the services must be included in the SA Intensive Outpatient Package. Many <strong>of</strong> these activities are reimbursable through Medicaid.The activities include but not limited to:a.
