1. It is expected that the transition planning for less intensive service will begin at the onset <strong>of</strong> these services. Documentation must demonstrate this planning.2. A consumer may have variable length <strong>of</strong> stay. The level <strong>of</strong> care should be determined as a result <strong>of</strong> consumers’ multiple assessments. It is recommended thatindividuals attend at a frequency appropriate to their level <strong>of</strong> need. Ongoing clinical assessment should be conducted to determine step down in level <strong>of</strong> care.3. Each consumer should participate in setting individualized goals for themselves <strong>and</strong> in assessing their own skills <strong>and</strong> resources related to sobriety, use/abuse, <strong>and</strong>maintaining recovery. Goals are set by exploring strengths <strong>and</strong> needs in the consumer’s living, learning, social, <strong>and</strong> working environments. Implementation <strong>of</strong>services may take place individually or in groups.4. Each consumer must be provided assistance in the development/acquisition <strong>of</strong> needed skills <strong>and</strong> resources necessary to achieve sobriety <strong>and</strong>/or reduction in abuse<strong>and</strong> maintenance <strong>of</strong> recovery.5. Substance Abuse Intensive Outpatient Package must <strong>of</strong>fer a range <strong>of</strong> skill-building <strong>and</strong> recovery activities within the program.6. The following the services must be included in the SA Intensive Outpatient Package. Many <strong>of</strong> these activities are reimbursable through Medicaid.The activities include but not limited to:a. Group Outpatient Services:I.Psycho-educational activities focusing on the disease <strong>of</strong> addiction prevention, the health consequences <strong>of</strong> addiction, <strong>and</strong> recoveryII.Therapeutic group treatment <strong>and</strong> counselingIII.Leisure <strong>and</strong> social skill-building activities without the use <strong>of</strong> substancesIV.Linkage to natural supports <strong>and</strong> self-help opportunitiesb. Individual Outpatient ServicesI.Individual counselingII.Individualized treatment, service, <strong>and</strong> recovery planningIII.Linkage to health carec. Family Outpatient ServicesI.Family education <strong>and</strong> engagementd. Community SupportI.Vocational readiness <strong>and</strong> supportII.Service coordination unless provided through another service providere. <strong>Behavioral</strong> <strong>Health</strong> Assessment & Service Plan Development <strong>and</strong> Diagnostic AssessmentI.Assessment <strong>and</strong> reassessmentf. Services not covered by MedicaidI.Drug screening/toxicology examinations7. In addition to the above required activities within the program, the following must be <strong>of</strong>fered as needed either within the program or through referral to/or affiliationwith another agency or practitioner, <strong>and</strong> may be billed in addition to the billing for Substance Abuse Intensive Outpatient Package:a. Community Support– for housing, legal <strong>and</strong> other issuesb. Individual counseling in exceptional circumstances for traumatic stress <strong>and</strong> other mental illnesses for which special skills or licenses are requiredc. Physician assessment <strong>and</strong> cared.
