Dare to Share Learning Events<br />
We have a number of methods for sharing<br />
learning following a clinical incident in order to<br />
keep our patients safe. This year we developed<br />
a new meeting to spread important safety<br />
information among a wide multidisciplinary<br />
audience and this has recently been shortlisted<br />
for has been shortlisted in the Clinical<br />
Governance & Risk Management in Patient<br />
Safety category of the Patient Safety Awards<br />
There have been four of these ‘Dare to Share’<br />
events held which have each focused on<br />
learning from adverse events which have<br />
occurred within the <strong>Trust</strong>, supporting us on our journey to deliver the best possible care. The staff<br />
involved in the incident share their own experiences and there has been an open dialogue about<br />
factors which contributed to the incident.<br />
The events have been organised by our governance department and supported by the Medical<br />
Director. Each event consists of presentations and we have focused on a wide range of topics<br />
including:<br />
Unexpected admission to ITU<br />
Use of an alarm system for monitoring patients’ heart rate<br />
Never Events<br />
Care of the patient treated with<br />
Non-Invasive Ventilation<br />
Safer sharps<br />
MRSA bacteraemia<br />
Mental Capacity, Deprivation of<br />
Liberty and Medication<br />
To date, the events have been attended by<br />
over 200 members of staff from a variety of<br />
disciplines all of who have been<br />
encouraged to share their thoughts on the<br />
trusts learning tree.<br />
During the meeting all attendees are<br />
encouraged to document the learning that<br />
they will take away and share with their<br />
colleagues. At the following meeting those<br />
who attend are asked to describe how they<br />
have change practice and what they have<br />
done differently as a result of the meeting.<br />
