Inhibiting CD26/dipeptidyl-peptidase IV intragraft activity attenuates posttransplant pulmonary<br />
ischemia-reperfusion injury after extended ischemia<br />
Objective: To investigate the effect of enzymatic DPP IV inhibition on ischemia-reperfusion<br />
(I/R) injury after extended ischemia prior to transplantation.<br />
Methods: A syngeneic rat orthotopic left lung transplantation model was used. In the control<br />
group (group I), donor lungs were flushed and preserved in Perfadex ® for 18 hours at 4°C and<br />
then transplanted and reperfused for 2 hours; Two treatment groups were tested: Group II<br />
donor lungs were perfused with and stored in 25umol/L Pro-Pro-diphenyl phosphonate<br />
(AB192), a small molecular weight DPP IV inhibitor; Group III donor lungs were only perfused<br />
with AB192, and not stored in the presence of the inhibitor. We measured peak airway pressure<br />
(PAwP) during the whole 2-hour reperfusion period, after 2-hour reperfusion, blood gas<br />
analysis was measured, and graft wet/dry weight ratio, myeloperoxidase activity (MPO) and<br />
thiobarbituric acid reactive substances (TBARS) were measured.<br />
Results: Grafts from group II, group III vs. group I showed a significantly increased oxygenation<br />
