Draft first regional report on the implementation of the Montevideo Consensus on Population and Development

This draft report seeks to give an account of progress in the implementation of the priority measures of the Montevideo Consensus on Population and Development in the region, as well as the differences between countries in terms of the degree of implementation. By highlighting relevant national experiences, it also seeks to facilitate the exchange of good practices among countries so that they can benefit from each other in their efforts to advance the implementation of the actions of the Montevideo Consensus.

Against this backdrop, <strong>the</strong> establishment <strong>of</strong> a public l<strong>on</strong>g-term care policy, c<strong>on</strong>sistent with priority measure<br />

31 <strong>of</strong> <strong>the</strong> M<strong>on</strong>tevideo C<strong>on</strong>sensus, is more important than ever. The reas<strong>on</strong>s go bey<strong>on</strong>d even dem<strong>and</strong>, as care<br />

for older pers<strong>on</strong>s can represent a growing c<strong>on</strong>tributi<strong>on</strong> to employment in many countries’ ec<strong>on</strong>omies. 71 In<br />

additi<strong>on</strong>, <strong>the</strong> almost complete lack <strong>of</strong> provisi<strong>on</strong> for this type <strong>of</strong> care in social protecti<strong>on</strong> systems may mean<br />

that older pers<strong>on</strong>s may not receive <strong>the</strong> help <strong>the</strong>y need in an appropriate, accessible <strong>and</strong> timely manner. The<br />

result is that a c<strong>on</strong>siderable percentage <strong>of</strong> <strong>the</strong> populati<strong>on</strong> is excluded from social services, <strong>and</strong> <strong>the</strong>ir assets<br />

<strong>and</strong> income may be jeopardized as <strong>the</strong>y seek help to carry out basic activities <strong>of</strong> daily living.<br />

A failure to capitalize <strong>on</strong> <strong>the</strong> current situati<strong>on</strong> <strong>and</strong> make changes will affect <strong>the</strong> possibilities for providing<br />

universal social protecti<strong>on</strong>. Unlike in 2013, when <strong>the</strong> M<strong>on</strong>tevideo C<strong>on</strong>sensus <strong>on</strong> Populati<strong>on</strong> <strong>and</strong> <strong>Development</strong><br />

was adopted, <strong>the</strong>re is now a legally binding instrument that facilitates its implementati<strong>on</strong>. The Inter-American<br />

C<strong>on</strong>venti<strong>on</strong> <strong>on</strong> Protecting <strong>the</strong> Human Rights <strong>of</strong> Older Pers<strong>on</strong>s facilitates <strong>the</strong> st<strong>and</strong>ardizati<strong>on</strong> <strong>of</strong> laws, policies<br />

<strong>and</strong> practices in States <strong>and</strong>, at <strong>the</strong> same time, helps to close gaps in <strong>the</strong> protecti<strong>on</strong> <strong>and</strong> enforcement <strong>of</strong> this<br />

group’s human rights. It also provides comm<strong>on</strong> definiti<strong>on</strong>s with a view to c<strong>on</strong>ceptualizing human rights in<br />

<strong>the</strong> c<strong>on</strong>text <strong>of</strong> an ageing populati<strong>on</strong>, as well as <strong>the</strong> opportunity to bring about a paradigm shift. Bey<strong>on</strong>d <strong>the</strong><br />

legal obligati<strong>on</strong>s it establishes, <strong>the</strong> C<strong>on</strong>venti<strong>on</strong> is an instrument for promoting effective public policies <strong>and</strong> a<br />

sound tool for implementing human rights <strong>of</strong> older pers<strong>on</strong>s by raising awareness at <strong>the</strong> nati<strong>on</strong>al level <strong>of</strong> <strong>the</strong><br />

resp<strong>on</strong>sibilities in respect <strong>of</strong> those rights.<br />

Lastly, a field that has not been developed in <strong>the</strong> same way as o<strong>the</strong>rs analysed in this secti<strong>on</strong> is palliative<br />

care. Despite <strong>the</strong> pressing need for it, <strong>the</strong>re remains much to be d<strong>on</strong>e. Palliative care c<strong>on</strong>sists <strong>of</strong> <strong>the</strong> provisi<strong>on</strong><br />

<strong>of</strong> <strong>the</strong> most basic care services. However, discriminati<strong>on</strong> against older pers<strong>on</strong>s has so entrenched n<strong>on</strong>-respect<br />

<strong>of</strong> <strong>the</strong>ir as dignity as human beings that even <strong>the</strong>y see it as natural <strong>and</strong> expect no o<strong>the</strong>r treatment in old<br />

age. Changing this percepti<strong>on</strong> <strong>of</strong> old age is <strong>of</strong> paramount importance to ensure that older pers<strong>on</strong>s can live<br />

aut<strong>on</strong>omously until <strong>the</strong> time <strong>of</strong> death, by respecting <strong>the</strong>ir wishes with regard to how <strong>the</strong>y die <strong>and</strong> providing<br />

all necessary support for a dignified death.<br />

For example, taking into account populati<strong>on</strong> ageing <strong>and</strong> <strong>the</strong> expected decline in <strong>the</strong> availability <strong>of</strong> family caregivers, it is estimated that dem<strong>and</strong> for l<strong>on</strong>g-term care<br />

in OECD countries will require a doubling <strong>of</strong> <strong>the</strong> number <strong>of</strong> l<strong>on</strong>g-term care workers as a percentage <strong>of</strong> <strong>the</strong> active populati<strong>on</strong> by 2050 (Colombo <strong>and</strong> o<strong>the</strong>rs, 2011).<br />
