Dinis da GamaClínica Universitária de Cirurgia Vascular do Hospital de Santa Maria, LisboaResumoDesde a descrição original em 1991 por Parodi que o objectivo do tratamento endovascular dos aneurismas da aorta eartérias ilíacas tem sido a prevenção da rotura, com menores riscos do que o tratamento cirúrgico convencional. Muito se temdiscutido sobre a exequibilidade técnica, durabilidade e vigilância, contudo pouca importância tem sido dada ao tratamentodas complicações agudas, nomeadamente ao tratamento cirúrgico convencional do aneurisma em rotura após tratamentoendovascular. Os autores apresentam o caso clínico de um doente submetido a tratamento endovascular de aneurisma da artériailíaca primitiva esquerda quatro anos antes, que acorreu ao serviço de urgência com um quadro clínico sugestivo de roturade aneurisma e fístula artério-venosa ilíaca de alto débito. O doente foi objecto de procedimento cirúrgico convencional, queconsistiu na ressecção parcial do aneurisma ilíaco e correcção da fístula arterio-venosa ilíaca.SummaryPost EVAR rupture of an iliac artery aneurysm complicated with an iliac arteriovenousfistula. Clinical report.Since the original description by Parodi in 1991, the aim of endovascular treatment of aortic and iliac aneurysms hasbeen the prevention of rupture, with fewer risks than the conventional management. Much has been discussed about itstechnical feasibility, durability, and follow-up, however little emphasis has been given to the treatment of its emergent complications,namely the aneurysm rupture after endovascular treatment. The authors report the clinical case of a patient thatunderwent endovascular treatment of a left common iliac artery aneurysm four years before, which presented in the emergencydepartment with symptoms suggestive of aneurysm rupture, together with high output iliac arteriovenous fistula. The patientunderwent conventional open surgical procedure, which consisted in partial resection of the aneurysm and repair of the iliacarteriovenous fistula.Volume XVIII - N.º 4231
