1. relaxarea musculaturii masticatorii: termoterapie<br />
cu căldură uscată pentru 20 <strong>de</strong> minute pe zi<br />
urmată <strong>de</strong> masaj manu<strong>al</strong> pentru 5 minute pe zi, cu<br />
scopul <strong>de</strong> a crește irigația sanguină <strong>și</strong> <strong>de</strong> a elimina<br />
reziduurile metabolice ;<br />
2. gimnastică mandibulară: <strong>de</strong>schi<strong>de</strong>rea gurii<br />
cu limba aplicată la nivelul p<strong>al</strong>atului pentru a facilita<br />
coordonarea musculară ;<br />
3. terapie funcțion<strong>al</strong>ă: terapie masticatorie <strong>și</strong><br />
fonetică timp <strong>de</strong> patru săptămâni, jumătate <strong>de</strong> oră<br />
pe zi.<br />
După 6 luni <strong>de</strong> urmărire a evoluției, <strong>de</strong>schi<strong>de</strong>rea<br />
gurii a crescut <strong>de</strong> la 25 mm la 40 mm, ără <strong>de</strong>viație<br />
<strong>de</strong> la linia mediană la <strong>de</strong>schi<strong>de</strong>rea gurii sau în<br />
protruzie, iar mișcarea <strong>de</strong> later<strong>al</strong>itate spre stânga<br />
a fost limitată.<br />
3. Discuții<br />
Datorită protecției arca<strong>de</strong>i temporo-zigomatice<br />
<strong>și</strong> a osului zigomatic, fracturile <strong>procesului</strong> <strong>coronoid</strong><br />
prin traumatism direct sunt foarte rare. Cu<br />
toate acestea, în acest <strong>caz</strong>, traumatismul direct<br />

cauzat <strong>de</strong> lovitura cu o cărămidă a condus la o<br />
fractură a apofizei <strong>coronoid</strong>iene drepte.<br />
Diagnosticul radiologic <strong>de</strong> fractură a <strong>procesului</strong><br />
<strong>coronoid</strong> nu este întot<strong>de</strong>auna ușor <strong>de</strong> pus <strong>și</strong> fractura<br />
poate rămâne nediagnosticată, datorită unor<br />
inci<strong>de</strong>nțe radiografice necorespunzătoare, sau a<br />
erorilor <strong>de</strong> interpretare. Odată cu apariția examenului<br />
CT, diagnosticul corect <strong>al</strong> unei fracturi a<br />
<strong>procesului</strong> <strong>coronoid</strong> nu mai este dificil. În <strong>caz</strong>ul<br />
prezentat, <strong>al</strong>ături <strong>de</strong> examenul CT, folosirea unei<br />
radiografii hemimandibulare a fost <strong>de</strong> asemenea<br />
utilă.<br />

4. Concluzii<br />
În concluzie, tratamentul chirurgic<strong>al</strong> a avut succes<br />
<strong>și</strong> datorită faptului că pacientul nu a prezentat<br />
modificări ocluz<strong>al</strong>e accentuate (având e<strong>de</strong>ntații<br />
parți<strong>al</strong>e întinse maxilare <strong>și</strong> mandibulare). Mișcările<br />
mandibulare post-operatorii au înregistrat doar<br />
schimbări minore. Alte tratamente (fizioterapie,<br />
exerciții mandibulare, terapie funcțion<strong>al</strong>ă, terapie<br />
fonetică), instituite imediat post-operator, au<br />
contribuit <strong>de</strong> asemenea la rezultatul fin<strong>al</strong> bun.<br />
