as it is a useful self-report instrument where the respondents were required to write their<br />

answers in response to questions asked. A total <strong>of</strong> 45 questions were included in the<br />

questionnaire. Both closed and open-ended questions were included in the questionnaire.<br />

Respondents were interviewed personally by the researcher. It took 3.5 to 45 minutes to<br />

interview one respondent The questionnaire was subdivided into eleven 11 sections as<br />

follows:<br />

SECTION A<br />

• Consisted <strong>of</strong>the demographic data. It was included for comparative purposes to find<br />

rehabilitation services each amputee requires.<br />

• Consisted <strong>of</strong> health services. This was included to determine the health services<br />

available in the community.<br />

• Consisted <strong>of</strong>health education for patients. This section was included to find out ifthe<br />

SECTIONE<br />

amputees were given enough health education.<br />

• Consisted <strong>of</strong> discharge planning. This section was included to determine if the<br />

amputees were prepared for discharge from the hospital<br />
