Lymphomas Therapy Success in the Treatment of Lymphomas F E A T U R I N G T H E B E S T O F G E R M A N M E D I C I N E الأ ورام اللمفية عالج ناجح لرسطان الغدد اللمفاوية Extensive research results after years of stagnation sor Hiddemann, Director of the Medical Clinic III at the Munich University Clinics. نتائج قيمة لبحوث واسعة بعد ‏سنني من الركود.‏ اأمراض الغدد اللمفاوية اجلريبية املتحوصلة.‏ A German study group has achieved a major improvement of the treatment of follicular lymphomas. The German Low Grade Lymphoma Study Group, GLSG, headed by the Oncologist Professor Wolfgang Hiddemann MD, was able to show that the combination of conventional chemotherapy according to the CHOP protoccol could be improved significcantly by adding the antibody Rituximab to the protocol. A prospectively randomized study of 428 patients from Germmany showed a significantly higher rate of response of 96% versus 90%. Above all, it also showed that the disease-free intervals and even the total survival time of the patients were longer. These research results, published in the online issue of BLOOD magazine, can be considered a milestone in the treatment of follicular lymphomas. After many years of stagnation the results of the German study group show that not only nearly all patients suffering from this disease can achieve temporary diseasefree periods, but also that the total survival time of patients with these kinds of diseases can be improved. “Germany is in a top position worldwide in the field of therapy research of follicular lymphomas and with this study it has again made an important contribution to the therapy development for these diseases,” points out Profess- Follicular lymphomas In this subgroup of Non Hodkin Lymphomas, the lymphomas correspond to cells of the lymph nodes (follicles). Subssequently, a swelling of the lymph nodes in the throat, armpit and groin region shows. In the western industrialized nations follicular lymphomas are the second most frequent subgroup of malignant lymphommas after chronic lymphatic leukaemia. In Germany, five to seven people per 100,000 inhhabitants get affected per year. Generally, people are affected between the age of 55 and 60 years. Both sexes are equally at risk. The original study was publlished in the following issue of BLOOD magazine: “Front-Line Therapy with Rituximab added to the Combination of Cyclopphosphamide, Doxorubicin, Vincristine and Prednisone (CHOP) significantly improves the Outcome of Patients with Advanced Stage Follicular Lympphomas as compared to CHOP alone – Results of a Prospecttive Randomized Study of the German Low Grade Lymphoma Study Group (GLSG)” Prof. Dr. Wolfgang Hiddemann (MD) Medical Clinic III at the Munich University Clinic Großhadern sekrmed3@med.uni-muenchen.de اإستطاع فريق بحث اأملاين طبي من حتقيق تقدم كبري يف عالج الأ ورام اللمفاوية حتت اإرشاف الربوفيسور فولف غانغ هيدمان اأخصائي الأ مراض الرسطانية يف جامعة غروس هادرن.‏ اأشار فريق البحث الأ ملاين لالأ مراض اللمفاوية ذات الإ نتشار البطيء )GLSG( باأن اإستخدام العالج الكيمياوي التقليدي حسب نظام )CHOP( يحقق نتائج ملموسة يف عالج هذه الأ مراض اإذا مااأضيفت له الأ جسام املضاده .)Rituximab( ويف دراسة اأجريت على ٤٢٨ من املرضى الأ ملان تبني حتسن نتائج العالج بنسبة ٩٦٪ مقابل ٩٠٪. وقبل كل ‏شيء اأظهرت الدراسة باأن مدة فرتة ‏شفاء املريض قبل الإ نتكاسة وطول حياته قد اإزدادتان.‏ و نرشت جملة ( )BLOOD على الإ نرتنت نتائج هذه البحوث التي تعترب دللة مهمة يف طريق الوصول اىل عالج ناجح يف اأمراض الورم اللمفاوي اجلريبي ‏)العقيدي(.‏ فبعد ‏سنني من الركود جاءت نتائج فريق البحث الأ ملاين لتوؤكد باأن كل مرضى الغدد اللمفاوية تقريباً‏ الذين يعاجلون بهذه الطريقة يحصلون على فرتة اأطول من اإختفاء املرض بالإ ‏ضافة اىل طول عمر املريض املصاب بهذا املرض.‏ ويقول الربوفيسور هيدمان،‏ رئيس العيادة الطبية الثالثة،‏ باأن اأملانيا تعترب يف مقدمة الدول التي جنحت يف عالج ‏رسطان الغدد اللمفاوية اجلريبية املتحوصلة:‏ اإن هذا النوع من اأمراض الغدد املفاوية،‏ اأو مايعرف Non Hodkin ،Lymphomas ينضاأ من اخلاليا يف الغدد اللمفاوية.‏ حيث تظهر اأورام هذه الغدد يف منطقة الرقبة وحتت البطني ويف املنطقة الأ ربية.‏ يعترب هذا املرض الأ كرث ‏شيوعاً‏ يف الدول الغربية بعد ‏رسطان الدم اللمفاوي،‏ حيث نسبة الإ ‏صابة به يف اأملانيا تصل حالياً‏ بني ٥ و ٧ من ...,..١ من عدد السكان بالسنة،‏ وتناسب اأعمارهم بني ٥٥ و ٦٠ ‏سنة من كال اجلنسني بالتساوي.‏ مت نرش هذا البحث مبجلة BLOOD كما يلي:‏ اخلط الأ ول من العالج بالريتوكسيماب Rituximab اأضيف اإىل خليط العالج الكيماوي التقليدي حسب بروتوكول CHOP املركب من ‏سيكلوفوسفاتيث )Cyclophosphamide( ودوكسوروبيسن ‏)‏Doxorubicin‏(,يف كرستاين)‏Vincristine‏(‏ وبردنيسون ،)Prednisone( وهذا يحقق نتائج جيدة للمرضى املصابني مبرض الغدد اللمفاوية املتقدم املقارنة باإستخدام الربوتوكول CHOP فقط،‏ حسب ما اأثبتته الدراسة الأ ملانية على جمموعة من املرضى.‏ بروفيسور د.‏ فولفغانغ هيدمان 27 27
