Bad Oral Hygiene can shorten your Life Studies show: patients without bacterial inflammations of the periodontium (periodontitis) live on average seven years longer. Periodontitis, commonly but wrongly called periodontosis, is a bacterial inflammation of the periodontium. Since this inflammation rarely causes any major trouble for the patients, it is often ignored. But this can have serious consequences. “This inflammation does not only cause the destruction of the jaw bone and the loss of teeth, scientists today also know of an interaction between periodontitis and general diseases that can limit your life,” says Professor Hickel. Patients suffering from advvanced stage periodontitis have a higher risk of getting a stroke, a heart attack or cardiovvascular disease. Bacteria and their toxins get access to the body. Additionally, the inflamed tissue produces so-called mediators. These are substances such as prostaglandin that report the inflammation to the body and cause appropriate reactions. If many teeth are affected, large amounts of these inflammation mediators can be produced and thus effect the entire body. The resulting defence reaction impairs varioous tissues and can increase disease risks. Causes Periodontitis is caused by bactteria-populated plaques located at the gingival margin, which lead to caries and gingivitis. A variety of about 500 different bacteria can be found in the oral cavity. ‏سوء العناية بنظافة الأ ‏سنان قد يقرص العمر بينت الدراسات باأن املرضى الذين ل يشتكوا من الإ لتهاب البكتريي للجهاز املثبت لالأ ‏سنان يعمرون مبعدل ‏سبعة ‏سنوات اأكرث.‏ اإلتهاب اجلهاز املثبت لالأ ‏سنان اأو مايطلق عليه عامة الناس اإلتهاب اللثة،‏ هو اإلتهاب بكتريي للجهاز املثبت لالأ ‏سنان،‏ و يهمل الناس هذا الإ لتهاب لأ نه ل يسبب اأمل كبري للمصابني به،‏ وهذا الإ همال ممكن اأن يكون له عواقب وخيمة.‏ ‏»فاإىل جانب مايسببه هذا الإ لتهاب من تاآكل لعظمة الفك،‏ وبالتايل ‏سقوط السن،‏ فان هناك تاأثري متبادل بني هذا الإ لتهاب وبني الأ مراض الأ خرى،‏ وهذا بدوره قد يقلل من عمر املرضى«.‏ حسب قول بروفيسور هيكل.‏ اإن نسبة الإ ‏صابة باجللطة الدماغية و السكتة القلبية اأو اأمراض الدورة الدموية عند املصابني مبرحلة متقدمة من هذا الإ لتهاب تكون كبرية.‏ فالبكترييا وسمومها جتري باجلسم بالإ ‏ضافة اىل ان الأ نسجة امللتهبة تفرز ما يعرف بالوسطاء ،)mediators( التي تكون وظيفتها اإخطار اجلسم بهذا الإ لتهاب و تفعيل املناعة لتقوم بدورها املطلوب.‏ اإذا كان عدد الأ ‏سنان املصاب بهذا الإ لتهاب كبري،‏ فاإن كمية كبرية من وسطاء املناعة ‏سوف يتم اإفرازها اإىل جميع اأجزاء اجلسم مما يسبب حتفيز جهاز املناعة و الذي يوؤثر بشكل ‏سلبي على اأنسجة اجلسم العديدة وبرفع نسبة خطورة الإ ‏صابة بالأ مراض.‏ الأ ‏سباب:‏ وتعود اأسباب هذا النوع من اللثة السليمة تكون وردية فاحتة اللون ليست حمراء منبسطة غري منتفخة والفتحات بني الأ ‏سنان تكون مملوءة.‏ Female patient (19 years old) with periodontitis. Dental Medicine Especially in the gingival pockets, where air is excluded, dangerous germs find ideal conditions to multiply. These germs can also be passed on to partners or children, e.g. by means of kissing or putting a pacifier in your mouth to clean it. In some cases this can also cause serious diseases. Therapy Gingival pockets of up to six millimetres (one to two milllimetres are normal) are usually treated conservatively, e.g. by means of mechanical cleaning of the root surface or rinsing. Deeper pockets often require surgery under local anaestthetic. If certain bone defects occur, the bone is restored by use of special membranes and proteins. “Long-term success can only be ensured through consistent oral hygiene by the patient and regular after-care by the denttist,” says Professor Hickel. Professor Dr. Reinhard Hickel (MD) Dentistry, Director of the Policlinic for Tooth Preservation and Periodontology, Munich University Clinics hickel@dent.med.uni-muenchen.de طب الأ ‏سنان الإ لتهاب اىل البكترييا املتواجدة يف اللويحات السنية التي تتوضع على اطراف اللثة و توؤدي اىل تسوس الأ ‏سنان واإلتهاب اللثة.‏ هناك اكرث من ٥٠٠ نوع من البكترييا التي تتواجد يف الفم لسيما داخل جيوب اللثة.‏ هذه اجلراثيم تتكاثر برسعة كبرية عند اإغالق الفم و عدم تنظيف الأ ‏سنان.‏ وممكن اأن تنتقل من فم لآ خر وكذلك لالأ طفال عند تقبيلهم اأو عند مص مصاصة الرضع لتنضيفها.‏ ولذلك يجب تنظيف املصاصة و عدم اإستعمالها قبال الكبار.‏ العالج:‏ اإذا كانت جيوب اللثة تصل حلد ٦ مليمرت من العمق ‏)السليمة تكون بني ١ و ٢ مليمرت(‏ يكون العالج بالوسائل التقليدية مثل غسل و حك ‏سطح اجلذر و الشطف.‏ اأما اجليوب العميقة فتحتاج اإىل اجلراحة حتت التخدير املوضعي.‏ فبعد فتح اجليوب،‏ تُفتح اللثة و يتم تنظيف ‏سطح اجلذر،‏ واإذا كان العظم مصاب فيتم هنا اإعادة بناء العظم وترميمه باإستخدام اأغشية خاصة و الربوتني.‏ ‏»لكن النجاح البعيد املدى ليتحقق اإل عن طريق العناية الدائمة بنظافة الفم،‏ و مراجعة منتظمة لطبيب الأ ‏سنان«‏ حسب قول الربفيسور هيكل.‏ بروفيسور وطبيب اأسنان راينهارد هيكل NEXT ISSUE No. 2 Be part of it !
