Youth Swim Lessons Swim Lesson Schedule 38 Youth Swim Lessons General Information: Due to many requests for those desiring to participate in this program, the City of Temecula will once again be offering an expanded youth swim lesson program through the months of May, June, July, August, and September. Lessons will be provided at the Community Recreation Center, Chaparral High School Pools, and Temecula Elementary School Pool. Please read the information below and refer to page 39 for further instructions and the “Mail-In Swim Registration Form”. Summer Swim Lessons Open Registration: Residents will be given an opportunity to request one swim lesson per child for any of the listed Summer Sessions during open registration May 5-11. Session 1 registration takes place during this time period as well. Once open registration has been completed, the other sessions will be available for mail-in registrations on the dates listed below. Swim Lesson Refund Policy There will be NO refunds after the first day of class. A $5.00 processing fee will be applied to all approved refunds made prior to the beginning of the first class. How to Register Residents: Mail - In Registration: We are proud to offer mail-in registration for City of Temecula residents only. It is important to keep in mind that mail-in registration will be accepted only during the weeks specified for the session indicated. Residents who use mail-in registration will be given first priority. Walk - In Registration: Walk-in registration will be available for residents who do not wish to use the mail-in process, and for non-residents, should class space be available. Proof of residency will be required at time of registration. Walk-in registration will be held at the CRC located at 30875 Rancho Vista Rd. from 4:30- 6:30pm on the dates indicated. Please remember that residents using mail-in registration will receive priority over walk-in registrations. The last day to register (if any openings are available after walk-in registration takes place) is by registering in person at the CRC no later than 5:00pm the Wednesday prior to the day of that lesson. Non-Residents: Walk - in Registration: Walk-in registration will be available for non-residents should class space be available. Walk-in registration will be held at the CRC located at 30875 Rancho Vista Rd. from 4:30-6:30pm on the dates indicated. Swim Mail-In Registration Form • Mail-in registrations will be accepted for City of Temecula residents only. Please be mindful that non-residents include but are not limited to residents of: Winchester, Murrieta, Anza, De Luz, Silverhawk, French Valley and other nearby communities. • Mail-in registration forms will only be accepted during the mail-in registration dates indicated on the previous page. All registration forms must be postmarked on the dates indicated for each session. Any forms received postmarked before or after the appropriate dates will be returned unprocessed. • A confirmation receipt with your class level, time and pool location will be mailed to you once your registration form is received and processed. Due to popular demand, please make sure that you have selected a first, second, and third choice on the registration form. Staff will do their best to accommodate first choice requests. • One registration form per participant is necessary. Additional forms may be copied, picked up at the Community Recreation Center during regular business hours Monday- Friday, 8am - 5pm, or downloaded from www.cityoftemecula.org • All swim lessons for Temecula residents are $45 each. One form of payment (check, money order, credit card purchase) per participant is required. Single payments for multiple requests (i.e. sibling groups) will not be processed. Cash may be used at walk-in registration only. Do not mail in cash. Make checks payable to the City of Temecula. If paying by credit card the payee’s name must match the name as it appears on the card. • If you would like to request placement with a sibling or other participant, please send all forms in the same envelope and we will try to accommodate your request. • The city may combine classes due to low enrollment. • Please mail the form below with your choice of payment to CSD, Swim Lesson Registration, P.O. LIABILITY WAIVER: I realize every precaution is taken to eliminate any injuries or hazards, and a competent supervisor is present; however, in the event of any injury, I hereby waive, release and hold harmless from any liability for damages for personal injury including accidental death, as well as from claims for property damage which may arise in connection with the above named activity, against the supervisor, the City of Temecula, its officers, agents, employees and volunteers. I further permit the use of activity/event photography and/or video for media promotion. In case of accident or other emergency, personnel of the Community Services Department and/or its agents are hereby authorized to secure medical care deemed necessary as a result of accident or injury for the participant.
