Performance Area 13: Permanency (i.e., the child exits the system with a connection to a<br />

permanent family).<br />

Measure: <strong>The</strong> percentage <strong>of</strong> children entering their 12th month in foster care during the<br />

previous 12 months who are permanent within 24 months <strong>of</strong> removal.<br />

Measure: <strong>The</strong> percentage <strong>of</strong> children entering their 24th month in foster care during<br />

the previous 12 months who are permanent within 36 months <strong>of</strong> removal.<br />

Measure: <strong>The</strong> percentage who exited to permanency among all former foster youth<br />

turning 18 during the period who were in care for 12 or more months at age 16.<br />

Measure: <strong>The</strong> percentage who exited to permanency among all former foster youth<br />

turning 18 during the period who were in care for 12 or more months at age 16 with all<br />

parental rights terminated.<br />

Measure: <strong>The</strong> percentage <strong>of</strong> children who are victims <strong>of</strong> substantiated maltreatment<br />

during the six months following discharge from foster care.<br />

Performance Area 14: adoption (including adoption failure rates)<br />

Measure: <strong>The</strong> percentage <strong>of</strong> children entering foster care during the previous 12<br />

months permanent within 12 months <strong>of</strong> removal.<br />

Measure: <strong>The</strong> percentage <strong>of</strong> children discharged to adoption during the previous 24<br />

months whose adoptions are reported as dissolved.<br />

IMPLEMENTATION PLAN<br />

On March 30, 2012, the first submission <strong>of</strong> the <strong>Pinnacle</strong> <strong>Plan</strong> was presented to the Co-Neutrals.<br />

Specific timeframes outlined in the agreement clarify the approval process with the Co-<br />

Neutrals and the Plaintiffs (see Appendix E). Estimated timeframes for implementation <strong>of</strong> the<br />

<strong>Pinnacle</strong> Point initiatives are included in Appendix G. While the plan moves through the<br />

approval process, OKDHS will begin an operational implementation plan and work with the<br />

<strong>Oklahoma</strong> State Legislature to secure funding (see Appendix H).<br />

<strong>The</strong> operational implementation plan will be a working document and will outline the steps for<br />

implementing the improvements detailed in the <strong>Pinnacle</strong> <strong>Plan</strong>. It will include such specificity<br />

as which staff members are responsible for certain action steps, specific timelines, and details<br />

for each <strong>of</strong> the initiatives. This effort will require child welfare staff at all levels, contractors,<br />

and external stakeholders to be engaged in the planning process. <strong>The</strong> operational<br />

implementation plan may also serve as the quarterly reporting document, provided the Co-<br />

Neutrals confirm it meets their expectations for formatting and content. By September 30,<br />

2012, OKDHS will have the first rough draft <strong>of</strong> this document completed; the rough draft will<br />

be revised upon the Co-Neutrals’ acceptance <strong>of</strong> the <strong>Pinnacle</strong> <strong>Plan</strong>.<br />
