must be based on the unique needs of the<br />
community. There is no correct number of formal<br />
members, but an HCC membership must include<br />
at least one general hospital or acute care facility.<br />
A single document that is signed by multiple<br />
organizations can constitute a formal agreement<br />
as long as the individual signing the document on<br />
behalf of the member healthcare organization<br />
(HCO) has the authority to make binding decisions<br />
and to commit the resources that may be called<br />
for in HCC response plans.<br />
The single HCC document (described above) may<br />
also make provision for sub-agreements or subcontracts<br />
(e.g., with the county coroner), to<br />
perform required <strong>HPP</strong> functions that are much<br />
more limited in scope relative to the overall HCC's<br />
responsibilities under the <strong>HPP</strong> cooperative<br />
agreement.<br />
Data Element #2:<br />
Has the HCC established a formal selfgovernance<br />
structure (e.g., By-laws for the<br />
board of directors and a charter that is<br />
multidisciplinary and representative of all<br />
members of the coalition)?<br />
Technical Assistance Guidance:<br />
The HCC governance structure must be<br />
described in a document that is referenced or<br />
embedded in HCC membership agreements<br />
signed by HCC member organizations. The<br />
governance structure described must describe:<br />
• Leadership roles within the HCC and the<br />
procedures for filling those roles<br />
• Decision-making processes<br />
• Process by which the governance structure<br />
may be modified<br />
• How the HCC Leadership coordinates with<br />
ESF-8<br />
Data Element #3:<br />
Please estimate the total percentage of the<br />
State population covered by each HCC within<br />
the State.<br />
Technical Assistance Guidance:<br />
The 'population covered' should be calculated<br />
as:<br />
a) The Total Population of the State, City, or<br />
Territorial Awardee, divided by<br />
b) The 'population covered' by the geographic<br />
area within which the HCC provides<br />
healthcare services, as determined by the<br />
most recent census estimates in effect at<br />
the time that the <strong>HPP</strong> grant was awarded<br />
(i.e., the 2010 Census would be used for<br />
each year of the FY2012-2016 grant).<br />
Data Element #4:<br />
Does the HCC include emergency management<br />
and public health as integral partners?<br />
Technical Assistance Guidance:<br />
For a ‘Yes’ to be scored, at a minimum, there<br />
must be ACTIVE and ENGAGED regular HCC<br />
member representation (apart from the HCC<br />
decision making processes) by the primary<br />
decision making representative from each local<br />
Emergency Management Agency (EMA) and<br />
each local Public Health Department (LHD) that<br />
exists within the geographic or jurisdictional<br />
boundaries for the HCC.<br />
• If representation does not occur on a<br />
regular basis, the HCC must have a written<br />
protocol that addresses how the HCC will<br />
interact with the primary decision making<br />
representative for each EMA and each LHD<br />
that exists within its jurisdictional<br />
boundaries.<br />
• In the case of the EMA, representation can<br />
occur through a liaison to the primary<br />
decision making representative.<br />
