What data must be reported?<br />

For each incident, planned event, or exercise<br />

reported on, please answer the following<br />

questions.<br />

1. The request for volunteers occurred during a<br />

[Check one of the following]:<br />

• Incident<br />

• Full Scale Exercise<br />

• Functional Exercise<br />

• Drill<br />

• Planned event<br />

2. This incident, planned event, or exercise<br />

utilized or demonstrated one or more<br />

function(s) within the: [Check one of the<br />

following]<br />

• <strong>HPP</strong> Volunteer Management Capability<br />

• PHEP Volunteer Management Capability<br />

• Both <strong>HPP</strong> and PHEP<br />

3. The type of incident, exercise, or planned<br />

event upon which the request for volunteers<br />

was based (check all that apply):<br />

• Extreme weather (e.g., heat wave, ice<br />

storm, etc.)<br />

• Flooding<br />

• Earthquake<br />

• Healthcare organization<br />

8. The date and time when volunteers were<br />

requested to arrive at staging area or on<br />

scene by health and medical lead<br />

9. The requested location for the deployment<br />

• Staging or assembly area(s) (not actual<br />

incident site)<br />

• Hospital(s)<br />

• Shelter(s)<br />

• POD(s)<br />

• Alternate care site(s), please specify<br />

10. The number of volunteers who were notified<br />

to deploy (“activated”).<br />

11. The date and time when the last volunteer<br />

was notified to deploy (i.e., “activated”).<br />

12. The number of volunteers who arrived at<br />

staging area or “on scene” within requested<br />

timeframe (numerator):<br />

a) Number of deployed volunteers<br />

registered in ESAR-VHP<br />

b) Number of deployed volunteers<br />

registered in other systems<br />

13. Date and time that last volunteer arrived at<br />

staging area or “on scene” within requested<br />

timeframe.<br />

14. Barriers or challenges to deploying volunteers<br />

to support a public health or medical incident<br />

within requested timeframe.<br />
