2. RISKS RELATED TO THE USE OF HAZARDOUS DRUGS<br />

studied and recognized to be effective (Wick, 2003; Spivey, 2003; Connor, 2002; Harrison, 2006; Au, 2006).<br />

Other systems are also available (e.g. Tevadaptor ® ). The cost-effectiveness ratio <strong>of</strong> closed-circuit systems or those<br />

limiting the risks <strong>of</strong> contamination (particularly in the context <strong>of</strong> Quebec health care) has not yet been determined.<br />

We are encouraging evaluative research to determine the role <strong>of</strong> this type <strong>of</strong> technology in Quebec practice.<br />

In a pilot study performed in 2007 with the INSPQ Toxicology Laboratory and CHU Sainte-Justine,<br />

environmental contamination levels below 1 ng/cm 2 <strong>of</strong> cyclophosphamide, ifosfamide and methotrexate were<br />

measured in most <strong>of</strong> the 75 samples taken from 5 sites outside the cabinet in a non-optimal environment without<br />

closed-circuit preparation equipment. Following the relocation <strong>of</strong> this satellite pharmacy, in June 2007, to an<br />

environment in keeping with the guidelines set out in this guide, without a closed-circuit system, new<br />

measurements confirmed the lack <strong>of</strong> environmental contamination in an equivalent number <strong>of</strong> samples. It is thus<br />

possible to significantly reduce environmental contamination through training and the use <strong>of</strong> various preventive<br />

measures (data on file, not yet published).<br />

How can the effectiveness <strong>of</strong> existing preventive measures be ensured? Regular environmental monitoring appears<br />

to be the best way to validate the preventive measures implemented. It should be stressed that there are currently<br />

no standards regarding drug contamination <strong>of</strong> the environment, nor any safe level <strong>of</strong> worker exposure. Stringent,<br />

regular monitoring <strong>of</strong> the work environment will at least allow the detection <strong>of</strong> any change in exposure levels and<br />

determination <strong>of</strong> the cause, in order to make the necessary corrections. The analyses proposed by the INSPQ<br />

Toxicology Laboratory will be useful in assessing the contamination by three products (see Appendix 5 and<br />

Chapter 15).<br />
