The importance <str<strong>on</strong>g>of</str<strong>on</strong>g> identifying people with L<strong>on</strong>g-term C<strong>on</strong>diti<strong>on</strong>s<br />

It is important to identify people correctly who have LTCs in order to ensure that they receive<br />

the right care in a timely manner. Receiving the appropriate treatment and management will<br />

slow disease progressi<strong>on</strong> and improve health outcomes. Early diagnosis is key to reducing<br />

early deaths from LTCs. There is increasing recogniti<strong>on</strong> that painful or disabling illness is a<br />

risk factor for suicide 15 , so attenti<strong>on</strong> to the mental health <str<strong>on</strong>g>of</str<strong>on</strong>g> people with LTCs is important.<br />

Why is it so challenging to manage L<strong>on</strong>g-term C<strong>on</strong>diti<strong>on</strong>s<br />

The definiti<strong>on</strong> <str<strong>on</strong>g>of</str<strong>on</strong>g> high quality care includes the need for that care to be safe, effective and for<br />

the pers<strong>on</strong> receiving it to have a positive experience <str<strong>on</strong>g>of</str<strong>on</strong>g> that care. Maintaining high quality<br />

care for LTCs lies at the core <str<strong>on</strong>g>of</str<strong>on</strong>g> the challenge particularly when many people have more<br />

than <strong>on</strong>e LTC.<br />

Box 3: Challenges to managing L<strong>on</strong>g-term C<strong>on</strong>diti<strong>on</strong>s<br />

• L<strong>on</strong>g-term c<strong>on</strong>diti<strong>on</strong>s can be complex and variable<br />

• Evidence based guidance and care pathways focus <strong>on</strong> single disease, however many<br />

people have multiple LTCs<br />

• Care needs to be co-ordinated across c<strong>on</strong>diti<strong>on</strong>s and across services<br />

• Having a LTC can be expensive for the patient, their carer’s and family<br />

• <str<strong>on</strong>g>Health</str<strong>on</strong>g> care resources are not unlimited; 70% <str<strong>on</strong>g>of</str<strong>on</strong>g> all healthcare spending is currently being<br />

spent <strong>on</strong> LTCs<br />

• The number <str<strong>on</strong>g>of</str<strong>on</strong>g> people with LTCs is increasing<br />

• The link between LTCs and poor mental health can complicate care<br />

• People with LTCs are more likely to have expensive unplanned admissi<strong>on</strong>s and planned<br />

care<br />

• Currently the majority <str<strong>on</strong>g>of</str<strong>on</strong>g> expenditure <strong>on</strong> LTCs is <strong>on</strong> acute care rather than <strong>on</strong> preventi<strong>on</strong><br />

Research and other work undertaken in recent years has broadened our understanding <str<strong>on</strong>g>of</str<strong>on</strong>g><br />

why the management <str<strong>on</strong>g>of</str<strong>on</strong>g> LTCs can be particularly challenging when individuals have<br />

multimorbidity. The findings from a Scottish Study can be transferred to our local populati<strong>on</strong><br />

to help inform understanding <str<strong>on</strong>g>of</str<strong>on</strong>g> the causes, needs and planning <str<strong>on</strong>g>of</str<strong>on</strong>g> high quality services 24 .<br />

The researchers c<strong>on</strong>cluded that “existing approaches need to be complemented by support<br />

for the work <str<strong>on</strong>g>of</str<strong>on</strong>g> generalists, providing c<strong>on</strong>tinuity, co-ordinati<strong>on</strong>, and above all a pers<strong>on</strong>al<br />

approach for people with multimorbidity". The Department <str<strong>on</strong>g>of</str<strong>on</strong>g> <str<strong>on</strong>g>Health</str<strong>on</strong>g> and the Kings Fund<br />

have produced a range <str<strong>on</strong>g>of</str<strong>on</strong>g> publicati<strong>on</strong>s that reflect a growing c<strong>on</strong>cern with how to provide the<br />

best care in the face <str<strong>on</strong>g>of</str<strong>on</strong>g> a growing issue <str<strong>on</strong>g>of</str<strong>on</strong>g> such complexity 16,17,18 .<br />
