EVIDENCE: Research<br />

Research focuses <strong>on</strong> the complex mix <str<strong>on</strong>g>of</str<strong>on</strong>g> factors that influence the development <str<strong>on</strong>g>of</str<strong>on</strong>g> l<strong>on</strong>g-term<br />

c<strong>on</strong>diti<strong>on</strong>s as well as finding better ways to diagnose and treat. It is equally important that<br />

research occurs locally, taking into account local cultural, social and healthcare settings.<br />

Local research, as illustrated in the case study, reviewed the impact <str<strong>on</strong>g>of</str<strong>on</strong>g> ‘pairs’ <str<strong>on</strong>g>of</str<strong>on</strong>g> LTCs <strong>on</strong><br />

hospital admissi<strong>on</strong>s 30 .<br />

Keele University is developing a risk stratificati<strong>on</strong> tool to identify ‘Frail Elderly’ people<br />

using informati<strong>on</strong> from primary care. It aims to allow clinical staff to identify and<br />

proactively manage ‘Frail Elderly’ patients, avoiding some inappropriate uses <str<strong>on</strong>g>of</str<strong>on</strong>g> urgent<br />

care services. People are defined as ‘Frail’ when they have an increased likelihood for<br />

death, disability, dependency, falls, and need for l<strong>on</strong>g-term care. Admissi<strong>on</strong> predicti<strong>on</strong><br />

and <str<strong>on</strong>g>of</str<strong>on</strong>g>fering preventive care can improve the health status <str<strong>on</strong>g>of</str<strong>on</strong>g> these high-risk patients.<br />

This may also reduce unplanned admissi<strong>on</strong>s making overall savings for the NHS. The<br />

tool will initially be applied to patients in nursing and care homes.<br />

Mosaic pr<str<strong>on</strong>g>of</str<strong>on</strong>g>iles <str<strong>on</strong>g>of</str<strong>on</strong>g> groups are also used to understand differing levels <str<strong>on</strong>g>of</str<strong>on</strong>g> hospital<br />

admissi<strong>on</strong>s and costs am<strong>on</strong>g people who have some LTCs. A local study looking at<br />

people with ‘pairs’ <str<strong>on</strong>g>of</str<strong>on</strong>g> LTCs, found that some combinati<strong>on</strong>s <str<strong>on</strong>g>of</str<strong>on</strong>g> LTCs had a higher<br />

likelihood <str<strong>on</strong>g>of</str<strong>on</strong>g> hospital admissi<strong>on</strong> that others. Over <strong>on</strong>e in three (67%) <str<strong>on</strong>g>of</str<strong>on</strong>g> individuals with<br />

both chr<strong>on</strong>ic heart failure and chr<strong>on</strong>ic kidney disease had at least <strong>on</strong>e A&E episode;<br />

four in five (80%) <str<strong>on</strong>g>of</str<strong>on</strong>g> this group had a hospital admissi<strong>on</strong>. In comparis<strong>on</strong>, over <strong>on</strong>e in<br />

three (37%) individuals with hypertensi<strong>on</strong> and diabetes had at least <strong>on</strong>e A&E episode;<br />

over two in five (44%) a hospital admissi<strong>on</strong>. The study also found higher costs<br />

associated with some LTC combinati<strong>on</strong>s.<br />

More recent research 31 focusing <strong>on</strong> primary care suggests the need to shift the focus from<br />

individual disease quality frameworks, particularly for patients with multimorbidity, to those<br />

using a range <str<strong>on</strong>g>of</str<strong>on</strong>g> tools to integrate and prioritise care pathways. The different evidencebased<br />

elements that c<strong>on</strong>tribute to the overall identificati<strong>on</strong>, management and improvement in<br />

care <str<strong>on</strong>g>of</str<strong>on</strong>g> people with l<strong>on</strong>g-term c<strong>on</strong>diti<strong>on</strong>s naturally work most effectively when combined in a<br />

seamless model <str<strong>on</strong>g>of</str<strong>on</strong>g> care such as the <strong>on</strong>e in figure 15.<br />
