A collective review <str<strong>on</strong>g>of</str<strong>on</strong>g> the nati<strong>on</strong>al outcome frameworks (NHS, <str<strong>on</strong>g>Public</str<strong>on</strong>g> <str<strong>on</strong>g>Health</str<strong>on</strong>g> and Adult and<br />

Social Care) dem<strong>on</strong>strated that there is a nati<strong>on</strong>al desire to the deliver integrated health and<br />

social care. The <str<strong>on</strong>g>Stoke</str<strong>on</strong>g>-<strong>on</strong>-<strong>Trent</strong> <str<strong>on</strong>g>Health</str<strong>on</strong>g> and Wellbeing Board has shown local commitment<br />

to the identificati<strong>on</strong> <str<strong>on</strong>g>of</str<strong>on</strong>g> priority indicators for individuals with LTCs. These priority outcome<br />

indicators are listed in table 6 and were mapped to the <str<strong>on</strong>g>Stoke</str<strong>on</strong>g>-<strong>on</strong>-<strong>Trent</strong> <str<strong>on</strong>g>Health</str<strong>on</strong>g> and Wellbeing<br />

Board Key Strategic Themes. This clearly highlighted a hierarchy <str<strong>on</strong>g>of</str<strong>on</strong>g> health and social care<br />

priority outcome indicators for <str<strong>on</strong>g>Stoke</str<strong>on</strong>g>-<strong>on</strong>-<strong>Trent</strong> that will promote and improve the health and<br />

wellbeing <str<strong>on</strong>g>of</str<strong>on</strong>g> individuals in our populati<strong>on</strong> with LTCs.<br />
