Carers and L<strong>on</strong>g-term C<strong>on</strong>diti<strong>on</strong>s<br />

It is important to recognise the unique c<strong>on</strong>tributi<strong>on</strong> <str<strong>on</strong>g>of</str<strong>on</strong>g> carers, families and volunteers who<br />

provide valuable services al<strong>on</strong>gside the care provided by health and social care<br />

pr<str<strong>on</strong>g>of</str<strong>on</strong>g>essi<strong>on</strong>als. With more care now provided in the community, carers are increasingly<br />

recognised as partners in care. It is crucial that they have the ability and c<strong>on</strong>fidence to take<br />

<strong>on</strong> the role and its resp<strong>on</strong>sibilities. There is legislati<strong>on</strong> in place which entitles any<strong>on</strong>e<br />

providing a regular and substantial amount <str<strong>on</strong>g>of</str<strong>on</strong>g> care, to have an assessment <str<strong>on</strong>g>of</str<strong>on</strong>g> their own<br />

needs.<br />

The Triangle <str<strong>on</strong>g>of</str<strong>on</strong>g> Care is a document originally published to support informati<strong>on</strong> sharing with<br />

carers in acute mental health services settings, but many <str<strong>on</strong>g>of</str<strong>on</strong>g> its principles are more widely<br />

applicable.<br />

Box 5: The Triangle <str<strong>on</strong>g>of</str<strong>on</strong>g> Care six key elements<br />

• Carers and the essential role they play are identified as so<strong>on</strong> as possible<br />

• Staff are ‘carer aware’ and trained in carer engagement strategies<br />

• Arrangements around c<strong>on</strong>fidentiality and sharing informati<strong>on</strong> are in place<br />

• A designated post resp<strong>on</strong>sible for carers is in place<br />

• A carer introducti<strong>on</strong> to the service and staff and informati<strong>on</strong> is available<br />

• A range <str<strong>on</strong>g>of</str<strong>on</strong>g> carer support services is available<br />

The local carer’s team <str<strong>on</strong>g>of</str<strong>on</strong>g>fer a range <str<strong>on</strong>g>of</str<strong>on</strong>g> support, including a Support, Time, Recovery worker<br />

for carers, and a time out service providing mutual emoti<strong>on</strong>al support and complementary<br />

therapies. Other support locally is available from the voluntary sector, for example, North<br />

Staffordshire Carers and local branches <str<strong>on</strong>g>of</str<strong>on</strong>g> nati<strong>on</strong>al organisati<strong>on</strong>s such as Age UK. The local<br />

team has also been doing work within the Combined <str<strong>on</strong>g>Health</str<strong>on</strong>g>care Trust to implement the<br />

Triangle <str<strong>on</strong>g>of</str<strong>on</strong>g> Care guidance. In recogniti<strong>on</strong> <str<strong>on</strong>g>of</str<strong>on</strong>g> the important role <str<strong>on</strong>g>of</str<strong>on</strong>g> the carer a Commissi<strong>on</strong>ing<br />

for quality and Innovati<strong>on</strong> (CQUIN) payment has been developed that rewards achievement<br />

<str<strong>on</strong>g>of</str<strong>on</strong>g> many <str<strong>on</strong>g>of</str<strong>on</strong>g> the elements c<strong>on</strong>tained within the triangle <str<strong>on</strong>g>of</str<strong>on</strong>g> care.<br />
