EVIDENCE: Pers<strong>on</strong>alisati<strong>on</strong><br />

Put simply, pers<strong>on</strong>alisati<strong>on</strong> refers to involving people with LTCs in decisi<strong>on</strong>-making about<br />

their care, moving them from being a passive receiver <str<strong>on</strong>g>of</str<strong>on</strong>g> care to a partner<br />

and an active participant in treatment<br />

decisi<strong>on</strong>s.<br />

It is estimated that a CCG covering a<br />

populati<strong>on</strong> <str<strong>on</strong>g>of</str<strong>on</strong>g> 100,000 could realise net<br />

savings <str<strong>on</strong>g>of</str<strong>on</strong>g> between £250,000 and £500,000<br />

using a patient-centred approach to angina<br />

A telehealth soluti<strong>on</strong> developed in <str<strong>on</strong>g>Stoke</str<strong>on</strong>g>-<strong>on</strong>-<strong>Trent</strong> is used al<strong>on</strong>gside risk stratificati<strong>on</strong> for<br />

supporting people with LTCs. “Florence” or “Flo”, is an NHS owned, low cost web<br />

applicati<strong>on</strong> designed to enable thousands <str<strong>on</strong>g>of</str<strong>on</strong>g> patients to take resp<strong>on</strong>sibility for the<br />

m<strong>on</strong>itoring and management <str<strong>on</strong>g>of</str<strong>on</strong>g> their own c<strong>on</strong>diti<strong>on</strong>. It allows multiple healthcare teams to<br />

share patient informati<strong>on</strong> and assist patients in the management <str<strong>on</strong>g>of</str<strong>on</strong>g> their own care. The<br />

model fits with pers<strong>on</strong>alisati<strong>on</strong> and QIPP, c<strong>on</strong>tributing to cost savings within the local<br />

health ec<strong>on</strong>omy. Flo is used to:<br />

• Regularly collect, m<strong>on</strong>itor and alert for example, blood pressure, weight<br />

• Improve the taking <str<strong>on</strong>g>of</str<strong>on</strong>g> treatment through encouragement, reminders and interactive<br />

Telehealth used in this way is especially helpful for people with LTCs, providing them and<br />

their carer’s with peace <str<strong>on</strong>g>of</str<strong>on</strong>g> mind. It can also help people live independently in their own<br />

home for l<strong>on</strong>ger and can avoid a hospital stay or delay moving into a residential care<br />
