New Zealand Standard on Risk
Management AS/NZS 4360, provides
Forensicare with a clear framework for
carrying out root cause analysis of critical
incidents. The assessment, which includes
methodology to identify and manage risks,
has been submitted to Council. The
methodology has been endorsed by
Council, and implementation across the
organisation has commenced. It is
anticipated that Council will formally adopt
the remaining components of the Business
Risk Assessment (risks identified) early in
2005-2006.
In line with recommendations included in
the Business Risk Assessment, the format
of the Forensicare Risk Register has been
amended to include greater detail in the
analysis and management of risk, and to
more comprehensively encompass clinical,
non clinical and corporate risk.
In respect to the delivery of our clinical
services, the assessment of potential risk
and management of risk for violence is a
priority area. On admission, information is
obtained concerning patients’ history of
violence, self harm, and related matters.
All direct care staff receive comprehensive
aggression management training at the
point of employment and thereafter
participate in refresher training three times
per year. This training is focused upon early
intervention, de-escalation, communication
skills and physical intervention techniques.
Staff training is also provided on the
‘Dynamic Appraisal of Inpatient Aggression’,
a measure developed by Forensicare to
assess patients’ ongoing risks of inpatient
aggression. This measure will be used in all
Forensicare acute inpatient settings. The
assessment of risk for violence is assessed
using a structured professional judgment
approach based on the HCR-20 Violence
Risk Assessment Scheme. These measures
are part of the Consolidating and
Strengthening Clinical Programs initiative.
Information from these measures is
employed in the development of risk
management strategies.
