Sabangan, Sinait, Ilocos Sur on or about 6:30 in the morning of January 28, 2010 conducted by
elements of Ilocos Sur CIDT led by PCI VILLAFLOR S BANNAWAGAN. Arrested suspect and the
confiscated items were brought to Ilocos Sur CIDT Office for documentation and proper
disposition while a case for Violation of Sec 1 of PD 1866 as amended by RA 8294 and
COMELEC Resolution 8714 were filed against the arrested suspect before the Provincial
Prosecutor’s Office, Vigan City, Ilocos Sur docketed under IS Nr. 03-INV-10A-00053.

2) Confiscation of One (1) cal .38 and one (1) Hand Grenade and the killing of
John Rey ANDRES @ Elias SALVADOR/ Menu, active members of ALMERON-ANDRES GROUP
during police operation by joint elements of PIB, Pangasinan PPO, S2 Occidental, Mindoro PPO,
Urdaneta City PS and Satt-3, Pangasinan CIDT, 3 rd COY RMG and PPSMC led by PSUPT RONALD
V GAYO on or about 11:15 in the morning of February 3, 2010 along Perez st., Poblacion,
Urdaneta City, Pangasinan by virtue of Warrants of Arrest issued by RTC, Br45 and RTC, Br 46,
San Jose Occidental, Mindoro in CC Nrs. R-6643, R-6649, R-6163 & R-6339 for Murder (4 Counts)
and Warrant of Arrest issued by RTC, Br51, Tayug, Pangasinan in CC Nr. T-4580 for Double
Murder with the Use of Unlicensed Firearm, but resisted arrest. Accused armed with cal .38 and
one (1) Hand Grenade exchange fire to the operating elements that resulted to his death
together with his unidentified companion. On the other hand, one member of the arresting
elements identified as PO3 Lamberto Ferrer was hit in his right leg during the shoot out and was
brought to Eastern Medical Hospital for medical treatment. Cadaver of the suspects lies at the
morgue of Amado Perez Emergency Hospital in Urdaneta City, Pangasinan.
3) Confiscation of One (1) cal .22 Magnum Revolver (Homemade) with
Unreadable SN; Eighty-two (82) rds Assorted Live Ammunitions; Twenty (20) pcs Slugs for cal .22;
One (1) Long Magazine for M16AR; One (1) Air Gun Rifle (Sky hawk) bearing SN: 0113 and the
arrest of William MARIA y Hector, Foreign National from New Zealand during implementation of
SW Nr. 15, S’ 2010 issued by Hon. Francisco A Ante, Jr, Presiding Judge, MTCC, FJR, Vigan City,
Ilocos Sur dated February 2, 2010 for Violation of RA 8294 (Illegal Possession of Firearms and
Ammunitions) at the house/premises of abovementioned suspect located at Brgy Paratong,
Sta Cruz, Ilocos Sur on or about 6:00 in the morning of February 11, 2010 conducted by joint
elements of Ilocos Sur CIDT led by PCI VILLAFLOR S BANNAWAGAN and Sta Lucia MPS. Arrested
suspect and the confiscated items were brought to Ilocos Sur CIDT Office for documentation
and proper disposition. Aforementioned suspect was put under arrest while waiting the result of
verification from the Bureau of Immigration as to the status of the above subject, since he is a
foreigner. A case for Violation of Sec 1 of PD 1866 as amended by RA 8294 were filed against
the arrested suspect before the Provincial Prosecutor’s Office, Vigan City, Ilocos Sur docketed

Series 80, SN: 220026 (Airsoft) and the arrest of Primo AGAS y Lorenzana during implementation
of SW Nr. 17, S’ 2010 issued by Hon. Francisco A Ante, Jr, Presiding Judge, MTCC, FJR, Vigan
City, Ilocos Sur dated February 5, 2010 for Violation of RA 8294 (Illegal Possession of Firearms
and Ammunitions) at the house/premises of abovementioned suspect located at Brgy Sevilla,
Sta Cruz, Ilocos Sur on or about 7:00 in the morning of February 11, 2010 conducted by joint
elements of Ilocos Sur CIDT led by PCI VILLAFLOR S BANNAWAGAN and Sta Lucia MPS. Arrested
suspect and the confiscated items were brought to Ilocos Sur CIDT Office for documentation
and proper disposition and subsequently transmitted for Inquest Proceedings before the
Provincial Prosecutor’s Office, Vigan City, Ilocos Sur docketed under NPS Nr. I-03-INQ-10B-

Hon. Ulysses Raciles Butuyan, Executive Judge, RTC, Br 51,
Tayug, Pangasinan dated February 9, 2010 at the
house/premises of abovementioned suspect located at
Brgy San Aurelio 3rd, Balungao, Pangasinan on or about
6:00 in the morning of February 12, 2010 conducted by
combined elements of SATT3, Pangasinan CIDT led by PSI
MELCHOR F PISO, PPPSM Tayug, S2 PPO, R2, Balungao PS
