significant proportion of facilities perform outreach activities aimed at improvingawareness of treatment options, there is still room for improvement.In terms of logistic and affordability barriers, many facilities report providing financialassistance for the direct costs of treatment in terms of reduced fees; however, reducedfees are often unaffordable to indigent clients. This raises a concern about theavailability of affordable treatment options to poorer communities. In addition, fewfacilities address the indirect costs associated with treatment entry, such as the costs oftransport and childcare services. Recommendations are made to address logistic barriers.These include shifting from facility-based outpatient service provision to the use ofmobile clinics located in disadvantaged, high-need areas. This will also help address thecosts of transport to facility-based services.In terms of linguistic barriers to treatment entry, the majority of facilities reportemploying multilingual staff and African language-speaking therapists. This study foundthat although all facilities report providing treatment programmes in multiple languages,a smaller proportion of facilities provide counselling that is culturally-appropriate and useculturally-appropriate assessment tools. This potentially inhibits Black/African personsfrom being retained in treatment.In terms of the appropriateness of treatment services, findings point to the need fortreatment facilities to receive ongoing training in order to ensure that their treatmentprogrammes are culturally, gender and age appropriate. Several recommendations aremade to increase the cultural, gender and age appropriateness of services.Findings point to the need for substance abuse treatment facilities in these provinces tointroduce routine, systematic client monitoring systems as well as the need for substanceabuse treatment programmes to be comprehensively evaluated. In addition, as part of themonitoring of the quality of substance abuse treatment services and the extent to whichthese services have transformed to address historical inequities in service provision, anational treatment audit should be conducted on a regular basis. Findings from thisnational audit should be used to inform decision-making about the allocation of fundingand other resources to existing facilities, based on the extent to which they provide8
services to historically underserved groups as well as the degree to which they providecomprehensive services of good quality.9
