ON PML/RARA t(15;17) FusionA structural rearrangement involving chromosomes 15 and 17in acute promyelocytic leukemia (APL) was first recognized in1977. The critical junction is located on the der(15) chromosomeand consists of the 5’ portion of PML fused to virtually all ofthe RARA gene. The PML/RARA fusion protein interacts witha complex of molecules known as nuclear co-repressors andhistone deacetylase. This complex binds to the fusion proteinand blocks the transcription of target genes. Other less commonvariant translocations fuse the RARA gene on 17q21 to thePLZF, NPM, NUMA, and STAT5b genes, respectively.The PML/RARA t(15;17) specific DNA probe is optimized todetect the reciprocal translocation t(15;17) (q24;q21) in a dualcolor,dual-fusion assay.ON MLL (11q23) BreakThe human chromosome band 11q23 is associated with a highnumber of recurrent chromosomal abnormalities includingtranslocations, insertions, and deletions. It is involved inover 20% of acute leukemias. The MLL (Myeloid-LymphoidLeukemia or Mixed-Lineage Leukemia) gene, named forits involvement in myeloid (usually monoblastic) andlymphoblastic leukemia, and less commonly in lymphoma islocated in the 11q23 breakpoint region.
