ATSG-NCSUBJECT: NCO Academy <strong>Policy</strong> Memorandum #4 - Commandant’s CriticalInformation Reporting (CCIR) Proceedings4. The responsible NCO will provide the findings to the Academy S-3 within 1 hour of theincident. Use the following format:a. Summary of incident:(1) Date and time.(2) Location.(3) Personnel and equipment involved.(4) Narrative report of incident circumstances.(5) Extent of injuries and property damage.b. Analysis of incident (will follow within 24 hours):(1) Unsafe actions/conditions identified.(2) Corrective actions necessary.(3) Remedial training required.5. The S-3 will provide the <strong>Soldier</strong> <strong>Support</strong> <strong>Institute</strong> G-3 and Commandant a copy of the CCIRNLT 2 hours of incident occurring.6.
