14.7 Private and commercial drivers with stage 3 or 4 renal diseaseApplicationAssessment guidelinesFitness guidelinesOSMV determinationguidelinesConditionsRestrictionsRe-assessmentguidelinesPolicy rationaleThese guidelines apply to driver fitness determinations for privateand commercial drivers who have stage 3 or 4 chronic renal disease.If further information regarding an individual’s medical condition isrequired, OSMV will request: a Driver’s Medical Examination Report, or additional information from the treating physician.If cognitive screening indicates that the cognitive functions necessaryfor driving are impaired, OSMV will not request further assessments.If the treating physician, or cognitive screening, indicates possibleimpairment of the cognitive functions necessary for driving, OSMVwill request a DriveABLE assessment.Individuals may drive if cognitive screening or, where required, acognitive functional assessment indicates that their ability to drive isnot impaired.OSMV may find individuals fit to drive if: the treating physician or cognitive screening does not indicatepossible impairment of the cognitive functions necessary fordriving, or where the treating physician or cognitive screening indicatespossible impairment of the cognitive functions necessary fordriving, a functional assessment indicates that they have thefunctional ability required for their class of licence held.No conditions are required.No restrictions are required.If the treating physician or cognitive screening indicates possiblecognitive impairment, OSMV will re-assess annually. Otherwise,OSMV will re-assess every five years or in accordance with theschedule for routine commercial or age-related re-assessment.Drivers with stage 3 or 4 chronic renal disease have a significant riskfor cognitive impairment that could impair their functional ability todrive.134
