CHECKLIST OF REQUIREMENTS<br />

This checklist is used at ward level to ensure that all important steps<br />

are taken and to document timing and responsible individuals.<br />

Patient’s death confirmed by doctor?<br />

Senior nurse in charge informed of patient’s death?<br />

Next of kin notified of death? (See breaking bad news guidelines)<br />

Procurator Fiscal Notification - inform as appropriate<br />

Death certificate prepared? (See instructions in Deaths Book)<br />

Death certificate given to family?<br />

Family returning later for death certificate? (if yes please record<br />

at the bottom of the page)<br />

Bereavement booklet given to family?<br />

Valuables/belongings returned to family?<br />

Valuables held in cashier office?<br />

Post Mortem If required - Patient’s family must sign<br />

Copy of signed post mortem consent given to family?<br />

Cremation Form B (if appropriate) completed?<br />

Cremation Form B (if appropriate) sent to Mortuary?<br />

Infection Certificate for undertaker sent to Mortuary?<br />

Consultant informed - within 24hrs<br />

GP contacted - within 24hrs<br />
