How to calculate SEWS Score<br />

• Do not add pain score to SEWS Score.<br />

• Record standard observations (RR, Sp02, Temp, BP, HR, AVPU).<br />

• Note whether observation falls in shaded “At Risk Zone”. Score as per SEWS key.<br />

• Add points scored and record total “SEWS Score” in bottom row of chart.<br />

• Action as per guidelines on front of chart.<br />

If RR >24 Review patient / CXR +/- gases / PEF (Peak Expiratory Flow) etc ✟<br />

If Sp0 2 sats 38 Blood cultures Other cultures Start antibiotic therapy if indicated.<br />

If Systolic BP130 Review monitoring (cardiac monitor indicated)<br />

IV Access<br />

Review patient / ECG / electrolytes ✟<br />

If responds to pain only<br />

or unresponsive Assess airway, BM, GCS, consider neuro observation chart, review patient / kardex.<br />
