Le samedi 9 octobre 2010, le monde entier célébrera la journée mondiale des soins palliatifs dont le thème retenu cette année est « Le partage du soin ». Cette journée vient nous rappeler que toute personne atteinte d’une maladie grave et au pronostic redoutable, a fortiori en phase terminale, a le droit de terminer son parcours dans la dignité, en recevant des soins anti-douleurs adéquats susceptibles d’améliorer sa qualité de vie. Au Maroc, force est de constater que, sur ce point, des efforts importants restent à faire pour en permettre l’accessibilité à tous. Les soins palliatifs ont été inscrits dans la Constitution de nombreux pays comme un droit fondamental de chaque personne malade mais, malheureusement, ce n’est pas encore le cas au Maroc. Cependant, plusieurs initiatives ont été lancées dans ce domaine chez nous, telles que la prise en charge, depuis plus de quinze ans, au CHU Ibn Sina de Rabat, des personnes atteintes de cancer ou non, mais souff rant de douleurs, ou encore la création du «Centre National d’Evaluation et de Traitement de la Douleur » à Rabat, en 2007, le premier du genre au Maroc et en Afrique ! De plus, dans le cadre de la politique de proximité des soins prônée par l’Association Lalla Salma de Lutte contre le Cancer, d’autres centres régionaux de traitement du cancer verront le jour afi n d’améliorer la qualité de vie des can- MOURIR DANS LA DIGNITÉ LE DROIT AUX SOINS PALLIATIFS cereux et de prendre en charge leur douleur. La sensibilisation des professionnels de santé, quant à elle, passe, pour le moment, par la « Formation des Formateurs », programme coordonné et parrainé par la faculté de médecine et de pharmacie de Rabat et la section marocaine de « Douleurs Sans Frontières », ONG internationale présidée par le Professeur Alain Serrie. Depuis 2000, ce programme a permis de former près de 280 médecins à la prise en charge de la douleur et des soins palliatifs. Cependant, d’énormes progrès restent à accomplir dans le domaine de la formation des professionnels de santé, en particulier dans la formation pratique des médecins. « J’espère qu’une journée nationale de l’accompagnement, qui coïnciderait avec la Journée mondiale des soins palliatifs, sera instituée pour permettre aux communes, hôpitaux, écoles, médiathèques et associations d’y participer dans l’objectif de rappeler la population à son devoir d’humanité et de solidarité à l’égard des plus vulnérables, dont les personnes âgées et les personnes en fi n de vie», signale le Pr Mati Nejmi, responsable au Centre National des Soins Palliatifs-Douleur et Directeur du Programme de Recherche «Douleurs Sans Frontières» au Maroc. À quand une loi garantissant l’institutionnalisation de la prise en charge de la douleur et l’accès aux soins palliatifs qui permettrait à tout un chacun d’accéder à des soins de support organisés et de qualité quels que soient la maladie ou le milieu social ? TRAITEMENT DE LA MALADIE DE PARKINSON PROGRÈS AU CHU DE RABAT Pratiquée dans quelques services de neurochirurgie très spécialisés en Europe et aux États-Unis, l’implantation d’électrodes (pour stimuler certaines zones du cerveau) est une technique de pointe qui permet d’améliorer de façon signifi cative et durable certains symptômes liés à la maladie de Parkinson, à savoir le tremblement, l’akinésie, la bradykinésie, la rigidité et les dyskinésies dopa-induites. Cette nouvelle technique a été récemment introduite au Maroc, au CHU Ibn Sina de Rabat, où deux patients en ont bénéfi ciées. Le chef de service de neurochirurgie au CHU Ibnou Sina tient à préciser que cette technique, une première au Maroc, concerne les malades ne répondant plus au traitement par médicaments. Elle consiste à fi xer de manière bilatérale deux électrodes cérébrales profondes, au niveau des noyaux sous-thalamiques. Ces électrodes sont reliées à un générateur d’impulsion (implanté dans le creux sous-claviculaire) et dont les paramètres seront par la suite réglés selon la réaction clinique et neurophysiologique du patient. Ces deux interventions, qui ont duré plus de 12 heures, ont nécessité la collaboration d’une équipe multidisciplinaire, à savoir des neurochirurgiens (du service de neurochirurgie de l’hôpital Ibn Sina), des neurologues, des neuroradiologues et des neuropsychologues (service de neurologie A de l’hôpital des spécialités) ainsi que des neuroanesthésistes (hôpital Ibn Sina). Ce traitement révolutionnaire dans la prise en charge de la maladie de Parkinson reste néanmoins coûteux : le prix d’acquisition des équipements nécessaires est de 250.000 DH environ et, à titre d’exemple, en Europe le coût de cette intervention équivaut à 50.000 euros !
