7. Name of the farm (in boroughs or towns, state street and number).
8. District -(Town).
9. When was the examination made by the Veterinary Surgeon.
10. On what has the diagnosis of the disease now furnished been based
(here state shortly, both the symptoms during life, as well as the
signs of disease discovered by a careful autopsy, and whether a
microscopcial examination has been made).
11. From where may the infection be presumed to have come.
12. Short report of what measures have been adopted to prevent the
spread of infection.

Note. A separate Schedule is to be employed for each different disease, and for each
different domestic animal. The Schedule shall be dated from the residence
of the Veterinary Surgeon.
N. B. This report is to be sent with the utmost dispatch direct from the Veterinary
Surgeon to the Chief of the Veterinary Department.
