one might as well be vaccinated when it is free of charge (the
Copenhagen regime; regime C)
The most important reason for not being vaccinated is that the person
concerned thinks that he is so healthy that he will be able to
overcome the disease himself.
When presented with a hypothetical offer of free vaccination most
of the elderly preferred to be vaccinated in general practice instead
of in a vaccination clinic. Nonetheless, 96 per cent of the respondents
in the municipality of Copenhagen in 1998 stated that they
were satisfied with the offer of free vaccination in a vaccination
clinic. Less than 1 per cent expressed dissatisfaction. Despite the
offer of free vaccination made by the local authority, 6 per cent of
the respondents who were vaccinated had chosen to pay for the
vaccination themselves, which was administered by their family
practitioner.
Elderly suffering from e.g. chronic diseases thought they were more
at risk than others if infected. In this group the vaccination rate is
higher and the elderly express more willingness to accept a hypothetical
offer of free vaccination in general practice or in a vaccination
clinic than elderly, who did not feel they were more at risk.
The Gallup survey concerning areas without structured activities
showed that the large majority of the respondents accepted being
contacted by a public authority or by their family doctor (hypothetically)
offering them to be vaccinated against influenza free of
charge. Surveys made by the municipality of Copenhagen regarding
1998 show that 66 per cent of the elderly over the age of 70
have actually accepted the offer of free vaccination under the local
authority vaccination programme.
Health-economic calculations
On the basis of the above-mentioned vaccination rates the following
economic calculations have been made:
Regime A; non-structured activities: The calculation is based
on a consultation fee of DKK 200 with the addition of the price
of the vaccine, DKK 58.75.
Regime B; general-practice vaccination; user fee: The calculation
is based on a consultation fee of DKK 150 with the addixix
