The calculations are sensitive to the following factors:
❖
❖
Remaining life expectancy. As mortality has not been proved
to be reduced after the epidemics it has been estimated that the
remaining life expectancy is equal to the remaining life expectancy
for the rest of the population. If the remaining life expectancy
is lower it becomes more expensive to save an additional
year of life.
Influenza outside the periods of epidemic. It is only possible to
roughly estimate the scope of the disease among the elderly
during influenza epidemics. Influenza, which may cause admission
to hospital and death and which may be prevented
through vaccination, is found in the population all the year
round. This may actually mean that the number of years of life
saved through vaccination is higher, whereby the cost per year
of life saved becomes lower than indicated in the table.
If regime C (the Copenhagen regime), i.e. public vaccination clinics
offering vaccination free of charge, could be established all over
the country, this would be the most inexpensive solution seen from
an economic point of view.
In socio-economic terms regime D2 (general-practice vaccination;
low rate) with a vaccination fee of DKK 94.63 would be superior
to regime A (non-structured activities) as it would be possible to
increase the vaccination rate significantly for the same amount.
The precondition of this is that an agreement is made between
Sygesikringens Forhandlingsudvalg (the Negotiation Committee
of Public Health Security) and Praktiserende Lægers Organisation
(the Organization of General Practitioners in Denmark). Increased
costs for home visits to the homes of immobile elderly persons have
not been included in the calculations. The Organization of General
Practitioners in Denmark estimates that approximately 5 per
cent of the persons over the age of 65 should be offered vaccination
in their own homes.
General assessment of the four organisational regimes and alternatives
Non-structured activities; regime A
❖ Based on telephone interviews (Gallup surveys) it is estimated
that approximately 40 per cent of persons over the age of 65
