The secondarygoal is for the applied medical therapy tobe well tolerated, not only locally in theeye, but also systemically, while the frequencyof application of eye-drops shouldnot overwhelm the patient. Consideringthe recommendations of the EuropeanGlaucoma Society (1), a principle aspect ofany glaucoma therapy is to minimize anyuntoward influence on the patient’s qualityof life, particularly so to maintain hiscompliance and motivation to continuehis therapy in daily life.All such aspects apply to any medical therapyand, thus, remain standard in InternalMedicine. In recent years, a new aspect inmedical therapy has developed in InternalMedicine, one which has barely been recognizedin Ophthalmology.It has been observed that certain key ingredientsnot only manifest their main effect,but also that they possess additionaleffects beyond their main mode of action,effects that were not taken into considerationwhen the therapy was originallyanalyzed and approved by governmentalagencies. Independent of the main mechanismof action, positively acting effects aretermed «pleiotropic effects» (meaning«multiple-acting») and represent a new researchsub-field in medical therapy.■ Pleiotropic effects in InternalMedicineFrom discussions on pleiotropic effects ofparticular ingredients in Internal Medicine,those which have predominantly beenfocused on are the statins. The main effectof statins is a reduction in the atherogenicLDL-cholesterine content. They inhibitthe enzyme that regulates the rate ofcholesterine biosynthesis, hydroxymethylglutaryl -coenzyme A (HMG-CaA)-reductase.Thereby, the synthesis of cholesterineis disrupted. Since the conversion of HMG-CoA to cholesterine occurs via intermediateproducts, statins also inhibit the formationof such intermediate products. Theseso-called «isoprenoids» are lipophilic moleculesthat play a role in various signalchains.For example, through the isoprenoidgeranylgeranyl-pyrophosphate, theprotein Ras is activated, one that not onlyinfluences the cyto-skeleton and smoothmusclecells, activates the plasminogenactivator-inhibitor-1(PAI-1), as well as theendothelial nitric-oxide synthesis (eNOS),but also inhibits the tissue-plasmino genactivator(tPA) (2). Additionally, the isoprenoidsexacerbate the oxidative stressand trigger inflammatory processes. Whenthese isoprenoids are inhibited through thestatins, then, in addition to a reduction incholesterine, the following positive additionaleffects occur (3):– amelioration of oxidative stress– inhibition of inflammatory cells– inhibition of local coagulation processes– inhibition of angiogenesis– stimulation of osteogenesis– inhibition of leukocyte adhesionand extravasation in infected regions.These additional pleiotropic effects haveproven to be positively additive therapeuticeffects, especially in the treatment of cardiovasculardiseases. Although almost half ofthe cardiovascular events occur in patientswith normal or even low LDL, it was possibleto demonstrate a cardiovascular risk reductionunder statins, independent of theprior LDL-status. A possible explanationfor this is the observation that statins – in-Pleiotrope Mechanismen der topischenGlaukomtherapieEin Aspekt wird in der Pharmakotherapie der innerenMedizin schon lange berücksichtigt, auch inklinischen Studien, findet in der Augenheilkundeaber erst wenig Beachtung: die pleiotropen Effekte,die man vereinfachend als «positive Nebenwirkungen»bezeichnen könnte (pleiotrop = «vielseitigwirkend»). Gemeint sind Wirkungen eines Medikamenten-Inhaltsstoffes,die über den eigentlichenWirkmechanismus hinaus eintreten. DieMedikamente wurden auf diese pleiotropen Effektehin ursprünglich weder untersucht noch entwickelt.Das Paradebeispiel der inneren Medizin sind dieStatine, die wirksam den atherogenen LDL-Spiegelsenken, um so das damit einher gehende Risiko fürAtherosklerose und ihre Folgen zu senken. Gleichzeitigsenken Statine aber auch den Gewebe-Plasminogenaktivatorund erhöhen die endothelialeStickoxid-Synthese. Damit und über weitere Mechanismenwirken sie sich auch unabhängig vomLDL-Spiegel positiv auf das kardiovaskuläre Risikoaus.Pleiotrope Effekte können auch bei Glaukommittelnbeobachtet werden.Betablocker entfalten antioxidative Effekte, diez.B. unter Timolol genauer beschrieben wurden.Weiterhin wirken sie auch neuroprotektiv, was beiBetaxolol gezeigt werden konnte. Diese Wirkungwird über calciumantagonistische und Natriumkanal-inhibierendeEffekte vermittelt.Weiterhin stimuliert z.B. Betaxolol die neuronaleStickoxidsynthese nach Ischämie und beeinflusstdie Katalyse des Neurotransmitters Glutamat.Auch für den Alpha-2-Agonisten Clonidin liesssich ein neuroprotektiver Effekt nachweisen.Carboanhydrasehemmer (CAI) wirken über komplexeMechanismen vasodilatierend. Zusätzlichwurde auch für Dorzolamid ein antioxidativer Effektbelegt, der sich im Trabekelmaschenwerk auswirkt– das könnte dafür sprechen, Dorzolamid bereitsfrüh im Glaukomgeschehen zu verordnen, umden antioxidativen protektiven Effekt lange auszunutzen.Interessant sind weiterhin antiapoptotischeEffekte der CAI.Antioxidative Wirkungen in Zellkulturen menschlicherTrabekelwerk-Zellen konnten auch unter Prostaglandinanaloganachgewiesen werden. Hiersind jedoch die Wirkmechanismen noch weitgehendunklar.Die Forschungen zu diesem Thema stehen erst amAnfang. Antioxidative und neuroprotektive Effektesind bislang am besten untersucht. Da in vielenFällen die Drucksenkung allein nicht ausreicht, umeine Progression des Sehnervenschadens aufzuhalten,ist es von grösstem Interesse, auch weiterewünschenswerte Effekte auszunutzen. Studienüber längere Zeiträume sollten diesen Effektenvermehrt nachgehen.
